People with a background in psychology can typically tell when someone's reaction is fake or real. I've called out staged prank videos before (in some the people involved eventually came clean about it, but this is rare), as well as real ones. Also, as a pointer, this assumption that if people prank each other a lot, they must be faking or surely they would know is an assumption not based on knowledge. They don't prank each other 24/7. The human mind cannot know when something is a prank or not no matter how many times that person has pranked them, unless the things they do are pranks more often than not being pranks. They might suspect it, but they cannot know for sure unless the pranker gives it away. As soon as a prank is over things go about as usual and the brain forgets that the next occurence can be a prank unless it is incredibly obvious. A boyfriend suddenly doing a drug deal is very plausible because sometimes you don't find something out about someone until some time after you've known them, or he could even be new to doing drugs.
