Ive finish threat lvl 5 twice, u sure if i enter for the 3rd time ill get the same completion reward?
100%!! You can choose the rewards you want along the paths (shards vs catalysts).

Yes, in July and again in August. There will be lots of places to get them. Incursions, Battlegrounds, specials quests etc. It will be a long 3 month process to get them all.
They refresh when the timer on them runs out

You should have 10 available in the Trader's Outpost. The rest will be in events and objectives over the next 3 months (June/July/August).
Where did you get the code nuber 4???
You get it from completing the side quest.
