The basic idea behind IVF is that the eggs are removed from the ovaries and fertilized outside of the body in the laboratory. There are two techniques for fertilizing eggs. The older method is called standard insemination. With this technique, a few thousand sperm are placed in a laboratory dish near the egg. The sperm have to penetrate the egg on their own. This method has largely been replaced by ICSI.
#ICSI stands for Intracytoplasmic Sperm Injection. With this technique, a single sperm is injected into a single egg. There are a number of reasons why ICSI is preferred over standard insemination.
First of all, if a man has low sperm counts or poor sperm movement then the chances are higher that fertilization will fail. Sometimes, standard insemination can fail even when the sperm numbers are normal. This is likely due to a specific problem with the sperm not being able to fertilize eggs on their own. Unfortunately, we are not able to predict ahead of time which men have sperm that might fail to fertilize an egg. ICSI bypasses those types of problems, so the chances of an IVF cycle failing because of fertilization failure is a lot lower with ICSI than with standard insemination.
When an egg is removed from the ovary it is surrounded by a dense group of cells called cumulus cells. These cells make it difficult to see the egg under the microscope. This makes it harder to determine if the egg is mature or not. Immature eggs cannot be fertilized. So when there is poor fertilization, it could be in part due to there being many immature eggs rather than a problem with the sperm
When we do ICSI, these cumulus cells are stripped away. This allows us to know with certainty whether an egg is mature and allows more precise evaluation of the egg retrieval.
Finally, ICSI is needed when we are doing genetic testing on embryos. With standard insemination, there may be sperm that didnt fertilize the egg still stuck to the outside of the egg after fertilization. This can cause an incorrect result when trying to do genetic testing of the embryo so ICSI is mandatory when doing genetic testing.

Hi Doctor Morris, thank you for making so informative videos and helping out the society. I wanted to know your words based on my data below for any suggestions-
Age 38, amh 3+, regular cycle and ovulation, 4 Failed IUI, 2 failed fresh transfer with ICSI,
2 nd ICSI was day5 transfer, took progestrone as suggested in ur videos alternate day injection plus daily thrice vagi.
Hysteroscopy + endoscopy done yesterday-- no scars found, tubes open.
Did blood test for both of us to find any existing genetic issues among us. No issues found. (Embryo genetic testing not available in our part of Germany).
Waiting for results for - Nk killer cells test & uterine plasma test.
We have 3fertilized eggs frozen on day1 left.

My IVF clinic did standard insemination of our eggs, then sent out for pgst. Is this wrong? We received a partial deletion of the 8th chromosome. I don’t know how many cells they tested. They retrieved 11 eggs, fertilized 5 and all but 1 arrested and the last one had that aneuploidy result. Sperm count/morphology results were good. Woman is 36 man is 32. IVF doctor is recommending sperm DNA fragmentation test, is that worth doing?

Hi Dr Randy. My wife and I have had a son who is now 4. We have been struggling for years to get pregnant. We have both done vitamins, diet and clomid treatment. The Dr says both are conditions are healthy. I have good sperm and she is also tested positive. Therefore we want do ICSI. Can you recommend anyone in Dubai as that is where we live. I wish I could travel to have you perform and assess us.

Women with uterine abnormalities have a higher risk for various problems such as miscarriage and preterm delivery. Some types of problems (like a uterine septum) can be corrected with surgery. Women with other problems such as a unicornuate uterus or bicornuate uterus in which one side has been removed are problematic since there i no way to surgically correct the problem. In many cases, use of a gestational carrier is recommended

I have heard the chances of twins are more in IVF....what about ICSI...IS THERE ANY SCOPE FOR TWINS?
Identical twinning is slightly increased in IVF pregnancies, even when transferring only a single embryos. I am not aware of any data that ICSI increases that risk further
