Its wonderful to see how standardize their training is across the coast guard that a guy from San Diego can be sent to Sitka and perform like he has been there for years. Many years ago I was the dedicated nurse that went on army medivac transfers of civilians from our area to larger cities with trauma centers and level intensive care nurseries. Often we had an issue where more senior hospital staff though they should actually be going, and maybe that was true from the stand point that they had a bit more experience in some of the treatments we had to do, but when the hospital was asked for a few volunteers to train with the medivac teams, who at the time did not often have corpsmen that they could take off duty at their base hospital, the idea was just what you see her, the fact the we could mesh and knew exactly what each were doing often without having to talk at all. Helicopter is a busy place with your flight deck right there and you often could not have a lot of medical chatter going on while the pilots were trying to communicate with each other and the base. 9 out of 10 flights, in the beginning, I had never seen or worked with the pilots or the airmen that came out to transport us, but it was so ingrained from training that it was like working with the same person over and over. This type of standardize training across the board is what saves lives in the long run.

I totally agree (remote living Alaskan-American)! I see nearly ALL 'western' countries do administer in-transit emergency pain relief and here's the USA- nope. AUS 'green whistles', UK 'gas 'n air', among the safe, benign, quick but effective methods. Is it that US gov't thinks ANY agent that delivers pain relief will turn (emergency!) patients into addicts?! How do WE change this??? We sure need to.
