It should be possible to create DEM map by hand. For model railroads maybe use the trackplan as a template. And draw your height map on top of it. And in HOG adjust the height values.
I figured it out, the problem was that I wasn't placing my saved .TGA file and my .GND file from my asset into the HOG folder on the desktop. After I did that, it worked just fine. Thanks for the tutorial.

You would be better off using Basemapz for creating a Trainz model railroad.
It seems the HOG program you linked is no longer able to be downloaded, at least the download links do nothing when clicked. Any suggestions?

every time i press generate GND in HOG, it says the blank tilemap .DAT file is blank or cant open. Any ideas?

Export DEM from .gnd file I think is not possible. You can take screenshots to make 2d map of route.
does it work for trainz 2011? also, is the map 1:1?
Yes I think so. Yes the map is 1:1 but be aware that it is not perfect.
