Shmi saved Padme's life and was open to helping even though she; was not obligated to help either. After the shock of all the events that happened on Coruscant & Naboo settled down; I think as a Queen & then Senator more effort could have been made on Padme's part.

I don’t remember what stopped her, but she also began freeing many other slaves on Padme’s orders and settling them throughout the sector around Naboo. She was Queen of a whole planet, and then Senator for it. The fact that she juggled that with spending her personal wealth to free slaves for multiple years and tried to go out of her way to free a specific slave is admirable. And Shmi lived a free life for her last few years anyways, happily married and loved. Anakin’s fall had many factors leading up to it from many sources, but the person who’d failed him the most was undoubtedly himself. He made the decision in the end to trust a man who he logically knew had plunged the galaxy into war and whose apprentice had endorsed attempts at assassinating Padme. Even without the benefit of knowing that Palpatine was faking being helpless any rational person would know he couldn’t be trusted by both physical evidence and logical deductions. Even says it in the Kenobi finale to the one person who would feel the most responsible for his fall: “You didn’t kill Anakin Skywalker; I did.”
I don't get how you could land anywhere other than on Anakin being the one who failed Anakin the most, because that is who is ultimately responsible for every single thing that he did.

Interesting points, but I think the one who failed Anakin the most was none other than Palpatine. Under the guise of a mentor and a friend, he manipulated him for his own ends, while Anakin trusted him.
Obiwan failed Anikan the most; he wasn't ready to be a mentor and was too loyal to the council to be a proper master, so Anikan found a father figure in Sidious
