Afib treatment using Tikosyn (Dofetilide) medication

  • There are many treatments available for atrial fibrillation, but none that I found to last a lifetime. I could've tried Afib surgery, but chose the medicine route on the advice of my electrocardiologist after Cardioversion procedure failed to sustain a sinus heart rhythm. If you are looking for afib treatment or considering Tikosyn (Dofetilide is generic), this video will give you a perspective from another fellow patient.
One important reason for the 3-night/4-day loading schedule is such that had I not converted by 2nd night, I would have gone through a cardioversion procedure to put me back into sinus rhythm with Tikosyn attempting to maintain that conversion.

I was leery of Tikosyn and refused it for months. Finally, I was in the hospital for a throat infection, and the EP doctors convinced me to take it, since I was in the hospital anyway. The next morning, anxious (they try not to show it) doctors awakened me and said that the med was not right for me, and there was a dangerous delay of signals between chambers of my heart.
I was in the hospital for 3 more days getting a magnesium drip. I survived the Tikosyn and an overdose of magnesium.
I should have followed my instincts!
It is good that it worked for you, though.

Been there, done that…. 48 yrs old, replaced mitral valve after being seen for a-fib. Over the years been cardioverted over 9 times. 2019 replaced tissue valve (bovine) with a mechanical valve. Went on TYCOSYN to control a-fib, after being on that for a short while, changed to FLECANIDE. 2020, got a pace maker, 3 months later got another cardiovert. I am now 67 yrs old and seem to be doing ok, just feel run down on some days. The pace maker keeps a history of heart activity. My last check up on the pacemaker showed occasional a-fib, but corrected itself on short order, no long term periods of time.
Path leading up to this in a nutshell: Wisdom teeth removal. Bacterial infection (unknown to me) traveled an settled in the heart, ate the mitral valve, 1 leaflet floating in the chamber, valve not working properly, doc said I should have been dead. Replaced the valve with a tissue valve, no blood thinners required so I could return to work in machine shop. Later (14 yrs) replaced with mechanical valve, blood thinners required for life, retired from machine shop.

Started when I was 48, am now 67, learned a whole lot in the last 19 years, good friends with the staff at the cardiologist office as well as the hospital, not a good group to be on a first name basis with, LOL, But on the other hand a good group to have as a friends on a first name basis, you have to be there to know what I mean!LOL.

First off, I hope everyone here is doing well. Please check with your physician and look at the Pfizer technical documents and studies for information. There is no "Loading" of dofetilde (Tikosyn), like there would be with Warfarin or Amiodarone. Tikosyn has a 10 hour half life. Your cardiac team will titrate your dosage after reviewing EKG's from 2-3 hours after dosing. If you look at the technical data on Pfizers site (not the consumer portion, but the health professional side), you can view appropriate information. Mostly trust your health care provider with the information they have given you. If you don't trust them then you should find someone else. Hoping a Normal Sinus Rhythm for all.

Thanks for the video. I also was put on Tikoyn 5 weeks ago, 1st 500mg then 250mg and then left the hospital on 125mg. I'm 68, had a new On-X mitral valve put in 6 months ago, the bovine mitral valve only lasted 4 years & then got calcified. I'm awaiting scheduling for an ablation next month. I've got to get this done before I have another stent in my left Circ. All 4 of my venous bypasses occluded after 3 years. Good luck - keep on biking.

Had I not been introduced to Tikosyn, I too might have gone through multiple ablations and complications. It's been a few weeks and Tikosyn kept me in Sinus rhythm without any side effects. Good life = time + money + health. Wish you the best outcome with ablation.

@@udtrader Thanks for taking the time to make this video. I've only just recently learned that I'm eligible for Tikosyn rather than ablation, and have had a hard time finding someone who has already begun the Tikosyn regime. I'm a healthy 67 year old man with no other complications than the reality of being 67 -- I don't run as fast, or swim as far, or lift as much, but I still do all three. Y0ur experiences in the hospital and your explanation of what. happened in your hospital stay are very helpful. Thanks a million.

Great video. I hope you are doing well. I suffer from atrial flutter and severe palpitations with atrial tachycardia. I Don't have Afib or any underlying heart condition. My pulse rate averages at 62 and blood pressure is on the lower side. I'm 53 and in good health, but these horrible palpitations are destroying my life. I started getting them at 34 from time to time, but now they are constant throughout the day. My doctor recommended Tikosyn. I have an appointment next month, but after reading the potential side affects it kind of scares me. I could try a beta blocker first. Your video help. Thank you for the insight.

Thanks for your story. I appreciate it and your positive outcome.
I started the Tikosyn (500 Mcg) loading in the hospital almost 4 years ago. It never converted my heart from a-fib to NSR. After the 3rd day my heart remained in a-fib and at that time my EP did a catheter ablation. The catheter ablation converted my heart into NSR.
I was told to continue taking Tikosyn 500 Mcg per day as it “would keep my heart in NSR”.
3 1/2 years of taking Tikosyn (I take no other medications and have no other health issues and am very active physically), my heart went back into a-fb (no apparent reason). My EP scheduled another catheter ablation and told me to continue taking Tikosyn 500 Mcg per day.
I think that I’m getting no benefit from Tikosyn and still have to deal with significant side effects (migraine headaches and vision disturbances).
Good luck and good health to you.

Thank you for this video. I hope this finds you continuing to do well on this drug. I, sadly, am not healthy. I am overweight and have other issues. I was converted in 2020 and put on cardizem, it didn't hold so I was put in the hospital and converted again and put on sotalol. I felt terrible on that drug, just terrible. so I went back on cardizem it is controlling my heart rate but not the rhythm.my doctor wants me to go on tikosyn. I am terrified. wondering if there are any side effects other than just having to be careful about the time span you take the drug. the rythmeol was mentioned but my doctor wasn't overly crazy about trying it. wonder what would be the benefit of tikosyn v. sotalol. I am scared. I am 56 and my father died young from heart issues and all of his sisters had a fib. now here go I. I just don't know what to do.......so scared

I'm still taking Tikosyn with 100% satisfaction and no side effects yet. For weight control, I walk a minimum 10k steps daily (walk to grocery) and stick to 16/8 diet schedule where I eat 8 hour window and DON'T eat for 16 hours. Easy to do - stop eating after 7PM and start eating after 11AM. Your body starts burning stored fat after 12 hours so 16 hours provide 4 hours of fat burning. Watch these two videos. Don't let fear consume you. Take control and start living. Only you can take action. Nobody else.

Thanks for making this video and sharing to our patients. This is what I do at least one of my patients in my everyday work and taking thus meds we watch QTc. intervals. we don’t want that long. If that’s too long, it’s gonna lead to flat line EKG.?so that is the reason if you double dose. it’s gonna prolong the QTc interval, and potential threat to life skipping is better than doubling if happen.
