    I do prescribe micronised progesterone to women with early pregnancy bleeding specially with recurrent miscarriages since a very long time. I am an OBG consultant in India. It has worked for these women with better outcomes- term livebirths.
    Thank you very much for your valuable research, Prof Arri. It is very helpful to me.🙏

    How much more effective are progesterone injections vs vaginal or oral pills?

    • @Prof_Arri_MRCOG
      @Prof_Arri_MRCOG  3 ปีที่แล้ว +2

      Hello, our clinical trials only studied vaginal micronized progesterone. The evidence from our studies therefore only applies to this vaginal progesterone.

    Has there been a new study for women with recurrent miscarriage with normal fetal karyotype that starts progesterone 3 days after ovulation?

    • @Prof_Arri_MRCOG
      @Prof_Arri_MRCOG  ปีที่แล้ว

      Hello, such a study is needed, but to my knowledge, I have not seen a randomised controlled trial on this question.

    Is prophylactic micronized progesterone recommended in pcod pregnancies in pts with long period of infertility treatment (eg.6 yrs) with no symptoms of bleeding sir

    • @Prof_Arri_MRCOG
      @Prof_Arri_MRCOG  2 ปีที่แล้ว +2

      Dear Basheera, no, in the context you have given, I would not generally use progesterone, but please note this is general guidance, and not specific to your care (for which you will need to seek a medical opinion). Thank you.

    This Prism trial infuriates me because the data does NOT help those that need progesterone starting 3 days after ovulation! For half of those women in the PRISM trial it was too late to start the progesterone only starting when they have bleeding. There is data of this but why on earth do doctors not investigate this further?

    • @Prof_Arri_MRCOG
      @Prof_Arri_MRCOG  ปีที่แล้ว +1

      Hello, I very much agree there is a need for a clinical trial to look at the value of luteal phase progesterone to prevent miscarriages. In the PRISM trial, we focused on the value of first trimester use of progesterone to prevent miscarriages.

    Dear sir, This is interesting. I have few questions.
    1. When suggesting a patient for vaginal progesterone, is there any possibility of increasing infections compared to oral progesterone?
    2. As infections are a known cause of second-trimester miscarriage, do you recommend doing high vaginal swabs or bacterial vaginosis screening prior to starting vaginal progesterone?
    Thank you.

      Dear Yapa, we didn't find any increase in infection in our two large trials - so this is reassuring. Thank you 🙏🏽
