There is no one "right" change process. It depends on your processes, your products, your people, your technology, and your supply chain. It's important to understand it's a series of trade-offs. For example, the more you control your process, the slower you are. The faster you are, the more likely you are to introduce errors. I don't know of any specific books, but the organizations that offer Configuration Management certification should have good resources.
