I feel sorry for her. Imagine what she was already going through with her husband and then this happens, and then on top of everything, nobody is taking responsibility and everyone is blaming everyone else. Poor woman. Hope she got some kind of resolution.
If you were travelling to or from work they your employer may have insurance to cover this, or the employer may have insurance to cover loss of earnings though injury whilst in service, similar to death in service cover. Worth discussing with your HR dept and union rep.

Never use a lift which is not level with the floor, it could be the brakes are not holding the lift securely
I would consider suing the manufacturer, thats a design fault, the door shouldnt open without the lift being securely attached to the floor level
The doors on almost all lifts can open when the lift is not perfectly level.

Will there not be CCTV in the lift that you could work out what time roughly it jammed
probably not from 3+ years ago. I doubt they would still have the recordings from that long ago.

Yes absolutely. The only thing you can't prove is when exactly they knew of the fault and when it happened.
