Its usually a sign the piece was made without too much planning and rather going with the flow. And the reason they are able to make that with far less planning (tho usually it is still iterative with sketches, clean up etc) is not because they are some god geniuses, but because they are pulling from experience and memory. That specific composition, expressions, pose, gesture in any given recent piece they must have measured, planned, sketched and cleaned tens if not hundreds of times, not to mention all the times they have not drawn, but seen and analyzed it.
