I have had the hardest time with Z codes in general just because I did not know when to apply them and how to look them up. Hello again! Another question I was doing a practice question and for one it was "Find the index location and code for: Personal history of malignant neoplasm of the lip." And for this question when I went back into the tabular there's two entries that to me seem almost identical Z85.818 Personal history of malignant neoplasm of other sites of lip, oral cavity, and pharynx Z85.819 Personal history of malignant neoplasm of unspecified site of lip, oral cavity, and pharynx. What the difference between "other" & "unspecified"? Apparently the answer from my book was "other" but I don't understand why they chose that one.

Hello I have a two part question about Follow Up. Example my encounter says Patient has abdominal abscess, the xray reports " No abdominal abscess Found". Thank you Brooke Johnson. I just learned injections myself. As for the Z-codes, I recommend taking some time to review the entire Z-code section in your book. You will be amazed what you find there and then if and/or when you need a code, you will remember seeing it in the Z-code section.

Muna, the Z-codes you listed above are "HISTORY" codes. Meaning the patient does not have today but had in the past or family had in the past, and these have a bearing on the current admission.

I agree P Makineni, syncopal episode will be the principal diagnosis. Since the electroencephalogram is scheduled to further clarify, then you can only code what you have which is the syncopal episode.
should I be studying the 2018 version of ICD 10 CM or the 2019 version I guess it should not make a different since you said the guidelines have not changed.
Brenda, if you have access to the 2019, I recommend staying current. But, up to the point that I am at, the rules from 2018 to 2019 have not changed. Now as we move forward, there are updates to the 2018 guidelines in the 2019 guidelines.
Hello Codemaster coach, What if the pt. had a surgery and then was admitted into the hospital to make sure everything went OK and no complications arise. Ie. gallbladder removal because of gallstones. Would you use the code for the gallstones as your principle DX. or would you use the Z48.xx code as the principal DX.?
Whatever diagnosis made the physician decide to admit the patient. If there was a complication, then that becomes the admission diagnoses. If no complication, why was patient admitted.
In this specific case, the pt. was admitted after the surgery for observation. That is why I thought Z48.816 would be the principal DX. But the feedback I am receiving is I need to make my principal DX. as the reason why the pt. came in for the surgery. I was just interested in your thoughts. On a side note, thank you so much for your videos! They are truly helping me understand and be a better coder!!
Wrong code for a two month old. That code is for a health exam of a newborn less than 8 days old. Did you mean your pediatrician put that for her diagnosis right after birth?

Hi there code master coach. I am taking a college coding class that's only 3 hrs a week. The prof. is nice but she moves very quickly. Your coaching is very helpful. So my question is- in the ISD -10-PCS there are "Root of Operations." Every time I tried to code them, I get lost. It seems hopeless for me to continue. Once I learn something and practice it, then it stays in my memory. Just having trouble understanding how to connect the correct root to the procedure. Any suggestions coach?
In terms of the ICD-10-PCS that's something that does not appear to even be covered on the CPC exam. If you buy the course through AAPC they don't require or include the ICD-10-PCS book as it's not covered in the training or on the test. The bundle just includes the ICD-10-CM, CPT and HCPCS.
At the clinic when a patient comes in for lab review I was told to use Z09 but, according to guidelines, it cannot be used as primary. I’m afraid my trainer is giving me wrong direction. I’d appreciate your help. Thank you.
MissHappiness36, after reviewing the Coding Guidelines for Z09, your trainer is correct. I understand what you are saying since the symbols state that Z09 cannot be PDX, but after reviewing the guidelines, 1.C.21..c.8, you can use Z09 in this case, but must also use additional code to identify any applicable history of disease code. (Note - patient won't have any other condition to code) (If patient does, then you should be using that code instead of Z09). Does this help?
It depends on where you live and what type of coding you do. New coders starting out here on the east coast start between $14 - $17 per hour. On the AAPC and AHIMA website they have salary tables.
Again, it depends on where you live and what type of coding you do. Most coders that I know get paid hourly. Companies sometimes pay per chart but most that I know get paid hourly.

What about Z00.110. My daughter is 2 month 6 days. The pediatrician put this on her papers for her diagnosis

Z codes are by far the most confusing so far.
Just think about when there is no "real" diagnosis of something "wrong" but yet the need for healthcare services.
