@@madisenhill960 What you want is a two-tier system or a fully private system with high levels of transparency. You don't want state-run healthcare believe me. I live in the UK and I'm originally from Portugal, both with basically fully run state care,. Not good. Most of the 1st world countries have a two-tier system and in the US you are not too far from it but it needs a complete overhaul. You have Medicaid and Medicare which is basically public insurance but not for everyone from what I know. There are many reasons why costs are so expensive however I believe the lack of transparency between hospitals and patients in terms of prices is one of the main reasons prices are out of control. You guys need to vote for people into power that can't be bought by big pharma and the corporations that run the hospital, only away for the system to change. And once again, I don't have to remind you that universal healthcare is not free, you will have to pay extra in taxes and potentially still have to pay for private insurance anyway (my case at least) unless you want to be one of the 7.5 million people currently on the NHS waiting list.
My mom is a Plastic Surgeon and the amount of times she's told me she has to fight insurance companies for her patients to be able to receive a life changing breast reduction is insane! Some of these women have such poor quality of life but insurance doesn't care it's so frustrating! I'm also a first year PT student and i'm starting to see these problems first hand so I totally get it!
Physical Therapist here. Thanks for not rushing to provide surgical intervention to every patient when PT can definitely help pain levels/ we can prolong them from having surgery!! Also really feel that the insurance model needs to be changed cause it’s impacting what interventions I can provide to a certain patient as well and dictates how much we get paid……
I’m usually frustrated when I am sent to physical therapy when they already know it’s a rotator cuff or a meniscus tear. Therapy for what? Repair it then give me therapy!

In these cases sometimes surgery isn’t always indicated/therapy can help maximize a patient’s strength and function prior to the operation to give them the best outcomes post-op!! I had a C5-6 disc implant completed in Oct 2022 for a disc herniation. I appreciated that my surgeon tried conservative measures for close to a year. I am so grateful for the expertise of my spine surgeon who understood how important it was for me in terms of hoping to get an implant and not a fusion. It really made me get such an interest in this medical area of expertise for those surgeons who feel this as their calling. I am so thankful for my spine surgeon with the Medstar organization in Wash DC. Thank you for all you do for your patients.
Unfortunately, insurance dictates many things which are not in the patient's best interest, nor for the best patient outcomes. I saw another video how a surgeon was describing how other implantable devices would work better, but the hospital and insurance companies did not care as it was costlier. And instead using inferior devices during surgery. Healthcare needs to shift back towards a more patient focused approach. I feel more people in healthcare need to speak out about this. Only way to make a change. Thanks for speaking up and sharing.
This will be an issue until we get away from our system. Even car insurance will do what they can to screw you over, the insurance industry as a whole is just toxic.

However, I think somewhere along the way, greed took over, it became too corporatized, and the true mission and vision, of helping others, of benefiting society as a whole, got lost. We can talk all about the negatives, critique, bash, but I think we need to focus on how can we change this to something more positive.

I give you so much praise for not wanting to perform surgery if you think other treatments can be effective. I had a L5S1 microdiscectomy a year ago. I couldn’t walk and my left leg was useless. I have reherniated the same disc and was having discomfort but not nearly as bad. My surgeon automatically wanted to do a fusion, no questions asked. I went to a physical therapist near me who reviewed my MRI and thought he could help me. 4 weeks later at 3 sessions a week, and continuing the exercises on my “off” days, I feel 100% again. Thank you for everything you do and having ethics behind your decisions, not all doctors do that. They only want to do what puts $$ in their pocket
I have massive respect for spine surgeons. I've had 2 microdiscetomies in the last 7 years. And it's a life changer. I went from bed ridden and lost 40lbs, to grocery shopping within a week of surgery.

Having an Insurance company dictating what kind care you can actually give to a patient is mind-blowing. For Dr's that got into medicine for the right reasons this must create quite a conflict.
Dr. Webb is absolutely right about the Insurance not paying them (Surgeons) to help out patients who are in need of Surgery. It happened to me 2 weeks ago when my insurance did not let the Surgeon to use this particular chemical (Correct me if I’m wrong if there’s a specific word besides chemical) for me during surgery so I ended up not getting the surgery and I’m paying shit load of money to them (My Insurance Company) every month. Sad world we live in!
Ive e been a nurse 20 years. I’ve worked with surgeons, pain specialist and many other specialists but many Insurance companies dictate the medical field. I hate it and even if the doctor against what insurance will cover and not cover the insurance has last call. It sucks. I’ve had many heated arguments with medical insurance companies but it’s all about money and cost. I’ve wanted to walk away many times from medicine because how medical insurance is run but I won’t because I love helping people and I’m good at it.

I'm 30. I was born with Pulminary Atresia. Let's just say I talk to my surgeon like my friend. I have had 5 open heart surgeries throughout my whole life. had had endocarditis twice in my life. had many different types of valves, thank goodness I live an hour away from Mayo in Rochester MN. My surgeon is actually written a book and asked if he can write about me in it!

UTSA has changed. I like that. Yea dealing with insurance is the worst part of medicine especially prior authorization on medications patients are already using.

Thank you for your outlook on insurance companies. We've dealt a lot with hospitals (not doctors) charge what they want and insurance companies pay what they want. Add to that the high cost just for the premium.
Same holds true for most insurance companies, sadly they are practicing medicine (Without) a license and dictating what your Doc is able to treat you with if you want them to pay. It is dangerous and should never happen!
You should start sending your patients to good chiropractors that do spinal curve correction and full spine adjustments plus decompression. Cracking people and sending them home isn’t enough. I had severe degeneration at L5 S1 and after a few months of chiropractic and at home therapy, my back is healing and moving again going from 15/10 pain to no pain and getting my life back. Very few people know and understand these different ways to heal the spine without surgery. It’s sad when I see people stuck in the medical system where nothing works and they get no information or real help. Usually it’s some kind of injection or nerve stuff not addressing the root problem. Each doctor is isolated from the other with no comprehensive approach. That was me until I found a spine center to actually show me how to do the proper therapies to get the correct curves in my neck and lumbar spine with x ray proof. I do specific exercises and stretching every single day at home, plus chiropractic, and I also did cupping at their practice. All of which provide permanent changes and I’ve been in no pain. I wish more people knew this.

It's important that you can recognize when you can't help someone or you'll find out how. My PCP did this when I asked some questions recently. Also insurance companies 🤬🤬
I’m sorry you have to spend time fighting with insurance companies for your patients health care and what you feel is in their best interest, on the other hand I would want to know if my surgeon was being denied access to whatever he or she felt was in my best interest. I applaud your efforts and anyone lucky enough to have you for a Dr is very lucky!
Insurance companies implement utilization reviews in which referrals for treatment are obtained and reviewed against generally accepted medical practices such as Office of Disability Guidelines and/or American Medical Association. If the treatment or referral is outside of these guidelines, then a peer review is usually warranted. This is where peer-to-peer phone calls are conducted with the peer doctor and treating doctor to better understand the reasons for the treatment in which Dr. Webb expresses his frustration about. Unfortunately, this can be a time consuming process, but insurance companies have the legal right to have a third party reviewer make determinations whether a treatment will be accepted and paid for by the insurer. Hopefully there will be a more streamlined process that allows for uninterrupted treatment.

Dr. Webb, I totally understand your frustration with the insurance companies. I sometimes wonder if I decided not to pay my insurance premiums on any particular month or months of the year (of course, not an option since it comes out of my paycheck automatically), because I did not need any medical services the same way they will deny coverage on certain procedures because they deem it not medically necessary even though you are in pain. I am sure they would not like it one bit. Sad fact is that you as doctor have to battle with these guys on a daily and us as patients have to do the same when we get sick or are in pain. Frustrating.
I agree with insurance companies dictating what a patient needs to recover! it's horrible that they dictate what ultimately ends up in their pockets at the end of the day

Ive e been a nurse 20 years. I’ve worked with surgeons, pain specialist and many other specialists but many Insurance companies dictate the medical field. I hate it and even if the doctor against what insurance will cover and not cover the insurance has last call. It sucks. I’ve had many heated arguments with medical insurance companies but it’s all about money and cost. I’ve wanted to walk away many times from medicine because how medical insurance is run but I won’t because I love helping people and I’m good at it.
I feel you brother, as a family medicine physician, we are the gate keepers for medicine but insurance is brutal in terms in dictating how we help our patients. Bless you

I feel your pain about insurance companies. I work in a law office and we focus exclusively on insurance coverage matters. They spend so much time denying claims to clients who pay millions of dollars every year for premiums. Your videos are very informative and I appreciate you sharing your side of the medical profession.

I'm 30. I was born with Pulminary Atresia. Let's just say I talk to my surgeon like my friend. I have had 5 open heart surgeries throughout my whole life. had had endocarditis twice in my life. had many different types of valves, thank goodness I live an hour away from Mayo in Rochester MN. My surgeon is actually written a book and asked if he can write about me in it!

Thank you for your outlook on insurance companies. We've dealt a lot with hospitals (not doctors) charge what they want and insurance companies pay what they want. Add to that the high cost just for the premium.
Same holds true for most insurance companies, sadly they are practicing medicine (Without) a license and dictating what your Doc is able to treat you with if you want them to pay. It is dangerous and should never happen!
You should start sending your patients to good chiropractors that do spinal curve correction and full spine adjustments plus decompression. Cracking people and sending them home isn’t enough. I had severe degeneration at L5 S1 and after a few months of chiropractic and at home therapy, my back is healing and moving again going from 15/10 pain to no pain and getting my life back. Very few people know and understand these different ways to heal the spine without surgery. It’s sad when I see people stuck in the medical system where nothing works and they get no information or real help. Usually it’s some kind of injection or nerve stuff not addressing the root problem. Each doctor is isolated from the other with no comprehensive approach. That was me until I found a spine center to actually show me how to do the proper therapies to get the correct curves in my neck and lumbar spine with x ray proof. I do specific exercises and stretching every single day at home, plus chiropractic, and I also did cupping at their practice. All of which provide permanent changes and I’ve been in no pain. I wish more people knew this.

It's important that you can recognize when you can't help someone or you'll find out how. My PCP did this when I asked some questions recently. Also insurance companies 🤬🤬

I’m sorry you have to spend time fighting with insurance companies for your patients health care and what you feel is in their best interest, on the other hand I would want to know if my surgeon was being denied access to whatever he or she felt was in my best interest. I applaud your efforts and anyone lucky enough to have you for a Dr is very lucky!

Insurance companies implement utilization reviews in which referrals for treatment are obtained and reviewed against generally accepted medical practices such as Office of Disability Guidelines and/or American Medical Association. If the treatment or referral is outside of these guidelines, then a peer review is usually warranted. This is where peer-to-peer phone calls are conducted with the peer doctor and treating doctor to better understand the reasons for the treatment in which Dr. Webb expresses his frustration about. Unfortunately, this can be a time consuming process, but insurance companies have the legal right to have a third party reviewer make determinations whether a treatment will be accepted and paid for by the insurer. Hopefully there will be a more streamlined process that allows for uninterrupted treatment.

Dr. Webb, I totally understand your frustration with the insurance companies. I sometimes wonder if I decided not to pay my insurance premiums on any particular month or months of the year (of course, not an option since it comes out of my paycheck automatically), because I did not need any medical services the same way they will deny coverage on certain procedures because they deem it not medically necessary even though you are in pain. I am sure they would not like it one bit. Sad fact is that you as doctor have to battle with these guys on a daily and us as patients have to do the same when we get sick or are in pain. Frustrating.

Also what about your computer, how were you taking the notes and how did you access all the patient scans remotely. Really cool. We met a while back at UTSA and now I’m a medical student in Nigeria. I agree so much about insurance Dr. Webb. I do not understand how and/or why they get to decide who gets what, when and how. This is so asinine to me and working in the hospital and seeing patient’s not getting the best treatment bc of insurance just irritates that absolute f**k out of me. Love the videos sir!
