- I was SO NERVOUS for my first day in the intensive care unit (ICU) - INTENSE is in the name!! I didn't know what to expect, didn't want to disappoint and certainly didn't want to make any mistakes!
The ICU is where some of the sickest, most unstable patients get transferred in the hospital. It's a place where patients can be closely monitored so that we can intervene when needed. The stakes are very high!

I’ll never forget my nurses and doctors when I spent 5 days in the ICU after a near death motorcycle accident. They were some of most caring and special people I’ve ever met. Thank you for all of your hard work, caring demeanor, explanations, and giving a face to the doctors who are seeing these patients. I was very lucky and thankful for my care. You are a very special person, especially for how busy you are and yet still able to film and take your time to share your days with us.

When I was pregnant with my son I was in ICU for 2 weeks. I had 12 abscesses on my brain. Which was causing my brain to swell and I had seizures. Long story but the staff in the ICU was amazing and they along with all the wonderful doctors saved my life and my sons.
Omgoodness. How did that happen? I had a bad abscess in my gum once that was so very painful. It was from a really decayed tooth. But I couldn’t imagine that on my brain. Glad your okay though
Thank you. I have two holes in my heart(as you can see I am not a doctor LOL) but when I go to dentist I have to take medication my OB/GYN at time told me I did not have to be premedicated because the odds of anything happening were so rare I would be ok(his words). I did not want to take medicine because I was pregnant and it made me ill. Got teeth cleaned. Started having severe headaches. Eventually went to ER for headaches they admitted me overnight and I had my first seizure. Did MRI. Saw all of the abscesses. Finally they learned it was because I was not medicated when I had my teeth cleaned and bacteria had settled in my heart and broke free and traveled to my brain causing abscesses. Took 4 months of IV meds to clear all of them. I suffer from Aphasia now. I am much better but no where what I was. My ICU staff was incredible and if it wasn't for a brilliant neurosurgeon who lives in my area(he was a consulting doctor when Christopher Reeve first was injured) I would not be here today. So bravo to any doctors & nurses. You have a forever fan!
Oh goodness! That had to of been scary especially being pregnant. It’s a miracle you and your little one is here. It’s always a blessing when hospital staff are great. Bc being in the hospital let alone the ICU is scary. And it helps when you have a supportive kind staff to get you through. .

I am a radiology student and I was surprised at how calm it was up in ICU. Everyone was so nice and helpful. You're right, it's definitely a team sport. All the nurses were there to assist with rolling a man on life support so we could get the cassette under him for a chest and KUB x-ray. So different than ER and general hospital ward!
As someone who has a major fear of hospitals and all things medical, I just want to say that it makes me feel so much better knowing that there are doctors like you out there! I love watching all of your videos :)

I also love that you ask if the nurses have any concerns because it just helps with communication. We don’t always like to blow up the pager but sometimes it is hard just to get a response so that’s such a good quality in a doctor, don’t ever lose that !
Love your vlogs! New grad ICU nurse here and one of my favorite things about the unit, like you said, is the teamwork. We all are so supportive of one another and I absolutely enjoy when the MDs take the time to sit down with me to talk about common diagnoses that we see on our unit/patho/and treatment. Keep up the awesome vlogs!!!

Good luck, Siobhan! You'll do great (and by the time you're reading this, I'm sure you already have). *My top tips for the ICU would be:* 1. the CVP monitor is your best friend, 2. never brush off an escalating pressor requirement, and 3. there are a million ways to keep yourself organized - pick what works with you and then stick with it :)
The Nicom cheetah monitor is far more accurate than a cvp for pressure management. If your facility does not have a nicom I would highly recommend looking into it. It’s noninvasive and can easily be used on a medical ward and possibly prevent many patients from requiring an ICU transfer and/or invasive lines.
I'm super late, but as an RN we hate CVPs. Lol, I understand the need for it but they're wildly inaccurate. You can get a fluctuating CVP on a cadaver, for reference.
The fact that you decided to be a doctor, makes you amazing! You are one of the best.
You are so very positive. I was glad to see the arterial line...sounds really scary. But, I know I'd always do better if the Dr. Were enthusiastic and calm and positive like you. You must be a blessing to your patients.
after spending 2 weeks in the ICU last year (at age 19), this video was surreal to watch. you seem to bring light, energy, and kindness to a really scary place. i would love to see more ICU related videos!!

You are an amazing doctor! I love your attitude, your willingness to work with the team, and take input from the other caregivers- especially the nurses- as an RN this is so important in being able to give the best care we can to our patients! There is nothing worse than a doctor who has an arrogant attitude! You are going to be great and I'm jealous of the staff that get to work with you!

When I was a nursing assistant, I sat with a 30 year old woman who had just had an aneurysm. It was quite intense because she was trying to yank all of the tubes out. She then went into my normal unit (neurology,) and I got to help her for the next month as she healed. It was great seeing her get discharged. She came up to me and thanked me for taking such good care of her. Thinking of her makes me wonder if I should have become a doctor like I planned, but then I enjoy what I do now better, I think!
Hi RainWanders - that sounds so rewarding getting to follow a patient through their recovery... that's something we don't get to often see while working in ICU - patients get discharged up to the ward and we don't always find out what happens to them! It's great to hear how much you care about your patients! Thanks for sharing!

I love your vlogs. I always wanted to become a doctor but life happened and I at least made it far enough to become a lab scientist. It’s really cool. But it is even more amazing to see what someone in your shoes deals with on a daily basis.

It’s so true that icu are the experts. I’m a step down nurse but every now and then I’m like ok this person needs more than I can give with 5 other patients and it’s so cool to hear about what happens when we send our patients off to the unit after an rrt.
I remember my ITU days! Long hours but loved it! And the satisfaction after getting an art line/central line or vascath!! Glad to see you like it!

I love getting a glimpse into Canadian medicine. It shows that socialized medicine is just fine. Yes, you may not have big, fancy facilities like we do here in Cleveland. Those don't effect the level of care. It's passionate Dr's. like yourself that make the excellent level of care.
I think it would be interesting to see how you work with the different professions at the hospital e.g. nurses, respiratory therapist, etc...

I had spent 1 month in the ICU at Detroit's Children's Hospital of Michigan MI due with heart infection I had. It was my longest ever in the hospital stay I ever had. Almost died during that time too so I had total of 5 open heart surgeries. I might probably have more in the future since my pig valve I have last 10-11 years they were inserted in 2020. Good thing about the renovated ICU in the 4th floor of Detroit Children's Hospital of Michigan is I had my own room instead of sharing with another patient which can be pain in the ass but part of getting some care from awesome nurses and doctors. A lot of my doctors and nurses I had are Catholic which is awesome.
My mother was admitted into a hospital near my home town, she would have four seizures a day and they didn’t know what would caused them so they sent her to fresno and had her in the ICU and omg those doctors and nurses were so nice even though they looked exhausted they always had a smile and the ones who came to the room when I called because my mother was having a seizure, she was amazingly nice and tried to calm me down because I tend to panic while she’s in it, but they calmed me down and did what they had to do and they helped my mother and i have a great amount of respect for those doctors and nurses out there who put their life on the line to save someone else’s.
My mother spent her last 2 days of her life in the ICU. She was breathing exactly like how you described during the second minute of this video. She had a pulmonary embolism. Love watching your videos, please keep it up....

Hi AVM - we didn't know it at the time, but the patient was bacteremic and his elevated respiratory rate was secondary to sepsis

I love the ICU, most nurses are are so friendly and don't mind helping out.
I’ve been binge watching these blogs and I’m like obsessed!! I love them and their so informative! And I 100% think you should do storytimes about crazy things you’ve seen or have been through working in so many different areas of medicine!! And just an FYI I hope you know that you’re amazing girl!! I mean as I said on the first video i ever watched (last night) I could never ever ever do anyyyyyythingggggg in the medical field!! Anything gross makes me nauseous, needles make me lightheaded (idk how I’ve ever gotten any tattoos or piercings) and blood.... even the word beyond grossed me out!! Now doing FX makeup, having pencils in eyes, plexiglass through heads, missing half your face .... love it. Fake doesn’t bother me even if it looks real asf cause I love doing it but real, no way can I handle that 🤢🤢🤢 so I give you doctors && nurses etc so much credit! I’d die if my life depended on doing even the most simple thing! And again I totally wish you lived in the states preferably eastern Long Island NY!!! I’d bring my son && daughter to you anyyyyytimeeeee!!

Seeing this made me happy that u explained how it kinda works in the ICU I'm from Australia and I have to much love for ICU stuff my aunty was in icu 3 times but she didnt make it back out after the 3rd time the staff was not only amazing at making her last couple hours as comfortable as possible for her but they helped support us while we said goodbye

I remember my first 24 call in ICU. After 6pm completely alone, no attending, no other residents, no interns. Was very nervous but thought, it cant be that bad on your first day. Well turns out it can)))) Got one septic patient. While I was trying to put a central line to start pressors another patient coded 7 times and died. Didnt know where to run what to do. At some point I think my spinal cord took over and it was all reflexes.
But its still the best damn profession in the world. No regrets

I genuinely wished someone like you worked in the ICU at my cities hospital when my mother was on life support. It would've made the tragic situation so much easier. 😕 the Doctors and nurses there.. While I'm sure are great at what they do and are only human.. Weren't that nice. Anyways.. You seem so wonderful! I adore how much you smile! 😊
At the beginning of the vlog you said you were nervous about this new experience, I think that is totally normal. Hey Siobhan! What do you do when you’re in the middle of your residency rotations and you realize you want to switch specialties? Like could you still change if you had found out you preferred emergency medicine over internal medicine during your EM rotation for example? Are there way to meet in the middle specialty-wise?
Hi Mariza! great question... I know it's possible to switch, although it's challenging (here in Canada it can be tied to the government funding that you get... i.e. if you are trying to switch from a 2 year program to a 5 year program it can be more challenging). It's certainly not impossible though! hope that answers your question!

I've been a patient in the ICU multiple times and boy it's not fun ... but it's interesting seeing it from a doctor's point of view, maybe next time I'm there (hopefully never) won't be so bad
This morning I visited a relative who is in the ICU and I was telling my family how watching your videos made me less anxious about visiting hospitals, such a coincidence to come home to this video. Even though it was a tough visit I definitely noticed there was a welcoming team atmosphere in the ICU.
