At 6:00 you start a new turn. You discard the brown moon and draw a nightmare. You resolve it by discarding the green key. You are left with 3 cards and you draw a nightmare. At that point you resolve the nightmare again. You don't put it to limbo and then shuffling it back into the deck.
I have the version from inPatience so maybe they added something that you don't have from your Z-Man Games version but the "Little Incubus" figurine actually can be used in Gameplay in three different ways to make the game: easier, gives you new options for your turn, "at a cost", and as an adversary where it "will stand against you."
