Came out in 1998 and depicts a fictional world war with China in 2006. GOG has it. Feels a bit eerie to play that game currently, though. XD

It's the most hassle-free option, as far as running it under Win10 and later is concerned - unless you're fine with using a ripped version from somewhere on the Internet. It's a game of the venerable "Panzer General" series, after all - just with (at time of release) modern hardware.

After going through Comanche games up to 3/Gold again recently, I tried Comanche 4. It runs far too fast, even with the supposed fix of forcing vsync on it. Also, it assigns throttle to the stick twist and rudder to the throttle, with no possibility to change that. Utterly useless.
warthunder will cost you so much money to get a helicopter. then you will relize that even with the first heli. it will take 2-3 week to get the newer heli. (with premium account that is) and then you will lose your premium account that came with the premium vehicle you bought, so now your forced to buy more premium account and now you spend 3-5 months straight up grinding in PvE helicopter battles witch IS SO BUGGED, several times i had tanks spawn under water and inside building? not even including you will average 10k research points every 1.5 hours WITCH IS CRAZY since you need 300k plus research points for a single helicopter. yeah sorry he didnt add your shitty pay to win game. (also who has time for a 1.5 hour match)

Btw, shouldn't your title include ' of the 21st century' because you've left out all the helicopter sims from the 20th century which include some of the best ever! You also left out Aero Elite Combat Academy for the PS2, which, whilst it is predominantly a jet fighter sim, does include the AH1S and AH64 helicopters and needs a mention here!
Had a *TON OF FUN* with Air Missions: Hind! Beat it nearly 100% complete twice by trying out different heli's. Hoping for a sequel!! 🤞
