It's because the story is written in kotoko's point of view. The author wrote the story as a record of his real life experience with her husband. His husband provided the guide in making the story. The feelings and the actions of Naoki here we're exactly her husband's feelings and experiences before they got married. Unfortunately kaoru tada the author of this manga series died in a illness and left her husband and daughter alone. Her husband didn't continue the story so it didn't progress any further. The ending should be kotoko dying years after she gave birth to her daughter just like kaoru tada
