Today I want to show you how to use the Abutment Screw Retrieval Kit from Nobel Biocare which has tools that help you remove broken or stripped screws from an implant.
00:17 The Simple Procedure
There are two different procedures to follow depending on the situation. The first is the simple procedure and this is for a case where the screw fragment that remains is rotatable. For this method, you will just need the appropriate abutment screw remover which can be found at the top of the kit.
There are three different screw removers and the one you will use is based on the size of the implant. There's one for 3.0 platform, one for narrow platform and finally one that works for the regular, wide and 6.0 platform implants. Each remover has laser markings on the side to indicate which platforms it's compatible with.
After you've chosen the correct screw remover you'll attach it to the handle which is located in the top left of the kit or you can attach it to a handpiece if you prefer.
You'll then need to place the end of the screw remover on to the screw and while applying light pressure rotate it counter clockwise. This will cause the teeth on the end of the screw remover to grab the screw and back it out. If you are using a handpiece make sure that the speed is set to a maximum of 50 rpm.
01:11 The Advanced Procedure
If you have tried the simple procedure but you aren't able to get the screw to rotate then you can use the advanced procedure. The first thing you'll do is select the appropriate rescue drill guide. There is a guide for each platform size of the Conical Connection, Tri-Lobe, and external hex Branemark implants.
Next, you'll attach your drill guide to the guide handle and use that to connect the guide to the interface of the implant. This guide will center the reverse drill on the screw and provide support while drilling. If the screw you're removing just has a stripped head and isn't broken you won't need to use the drill guide and instead will use the reverse drill free hand. There are just two reverse drills, one that works for the 3.0 and narrow platform implants and the other that works for the regular, wide and 6.0 platform implants.
Connect the reverse drill to your handpiece and set your handpiece to reverse mode with a speed of 2000 rpm. Place the reverse drill in to the drill guide and drill about 1mm in to the screw using irrigation and periodic air-blasts. When complete, you can remove the drill guide from the implant.
You'll now need to use the screw retrieval instrument to back out the screw. Note that the screw retrieval instrument is not the same as the screw remover that was used in the simple procedure earlier. Choose the appropriate size and attach the screw retrieval tool to the short handle. Place the tool in to the previously drilled hole and rotate it counterclockwise with light pressure. If the screw still won't rotate you can attach the retrieval tool to a torque driver and try again.
02:39 Evaluate & Repair Threads
Once the screw is removed you'll want to evaluate the internal implant threads for damage. The easiest way to do this is to take an unused screw, and slowly screw it in to the implant. If you encounter any resistance you may want to use the screw tap repair tool which is located at the bottom of the screw retrieval kit. There are 5 different thread sizes used in Nobel Biocare implants and you'll need to contact tech support to determine the thread size of the implant you are trying to repair.
Once you know your thread size you can attach the appropriate tool to the short handle. Place the tool in to the implant and turn clockwise to repair. - วิทยาศาสตร์และเทคโนโลยี

I had a Nobel implant patient come in with an abutment in place. The screw holding down the abutment had its head stripped. It was a knock-off screw. Maybe this kit will work.
Yes, this kit will work on nobel implants regardless of the screw type. With the abutment still in place, the simple procedure with the "abutment screw remover" can be used. The advanced procedure could only be used if no abutment was present.

The Bio3 implants are not sold in the US so I'm not very familiar with them. Based on the design there is a chance that the Astra/Hiossen screw removal tools from the Salvin kit would work. www.salvin.com/Salvin-Implant-Rescue-Kit-pluIMPLANT-RESCUE-KIT.html

once an abutment has been inserted into an implant and torqued to 35, can it be easily removed without damaging or loosening the implant? is there risk? thanks!
Yes, the abutments can easily be removed without damaging or loosening the implant.

Your regular dentist should be able to either help you or refer you to someone else who can. Otherwise, you can search for a Prosthodontist, Endodontist, or Implant Surgeon in your area.
How can the abutment screw restored in case of unknown manufacturer?
That can be difficult. If you're able to remove the screw, a lab may be able to identify the type of screw just by looking at it. You can also take a radiograph of the implant and use an identification service to determine what it might be. Knowing what system the implant is will typically help in determining what type of screw it is.

I'm concerned about implant screws from China that look like standard wood screw thread. They are listed as titanium but look like stainless . They say FDA approved but I found out many electrical items that have a CE approval stickers are not Certified but instead China claims it stands for China export !
Show me how to fish instead of giving me fish when I am hungry. In other words find out how and why implant abutment screw broke and engineer to minimize the occurrences. Please do not say the patient did not keep his or her implant clean.
