There shouldn't be any problems with inspecting or equipping the default knife, some people just don't have money or just like the default knife. But inspecting or equipping it in open spaces like running down mid or closed places like connector is a reason to assume he is cheating.

There's an "automated overwatch" system that kicks in if someone gets red trust (uses a shit cheat that modifies the game in ways that are detectable by reading hooks instead of using VAC itself). That system usually has a delay as it needs time to read match data, which is why he didn't get banned instantly.
they don't have manual ow system yet but they do have an AI for that so you'll get an OW ban if you do "cheating behaviors" like setting your mouse sensitivity really high
