Another aspect to consider is the fact that Tusk clearly needs Van Horsing alive, not only because she just traps him, not kills him, but she even goes out of her way to use the alter to remove his curse first.
I'm a year late, but I think this is actually because Tusk uses Van Horsing as the base to clone her bodyguards from. they look nothing like the shopkeepers, but they have the same brow ridges, chin cleft, and nose as the original Van Horsing.
Some notes: -She does not care if you killed Vlad. You can even wear the cape in front of her -The bodyguards are most likely well paid hired hands, based on the bestiary -The royal jelly in her palace is probably from Kali, as is Van's curse removal

The ghost is Yama's dead wife. That's why the empress statue shows up in eggplant world and why the ghost is connected to it. Why she's a ghost haunting the adventurers, I don't know.
i would like to take a little side note i noticed, i could be wrong, however her broch (or whatever its called) looks oddly simular to the charm in spelunky hd that if i remember correctly. makes you imune to lava.

When you encounter him in volcana and he says “Tusk won’t be happy about this, but thank you.” I think he was locked up by Tusk for whatever reason and since you let him out she wont be happy with the player, i also think Van Horsing kills Vlad to pay back the player and obviously because he is a monster hunter. I also thought she locked him up again because he escaped.
maybe van horsing got uncursed when madame tusk tried to get the kapala but if you get 16 points of favour with a curse player it on curses them instead of getting a kapala
I just want to point out (if anyone hasn’t already) that “madame” is not pronounced “madam aye” but is instead just “madam”, like how you refer to a woman politely. “Madame” is the French equivalent of “madam” in English and should be pronounced the same. Sorry if I sound critical, I just noticed and it bothered me.
I always thought the reason van horsing has the alien compass is because he is actually van helsing and is the vampire hunter. And it makes sense because the alien compass is proven to work in between the fabric of reality and its model clearly shows a radar so i assumed he used it to track down vlad
I see another part to this which would be Van’s curse. Curses can notably only be removed by elixir. In the elixir journal entry, it says it is made with royal jelly. This would mean that Tusk sacrificed enough people for the jelly, to then make the elixir to cure Van. She might have done this to squeeze information about Vlad from him. After all, why go through hundreds of lives and use your elixir only to make sure your prisoner lives? It doesn’t make sense. I mean I’m probably wrong about this but still.

I heard a similar theory that tusk is also a vampire, w the big tusks and the necklace she has looks similar to Vlad's amulet from HD. Would also explain why she would imprison horsing too
neat little side note I'd like to mention is how the Olmec curse and temporal loop paused after the canonical defeat of Yamaha and seems to start up again on the current setting; however this implies that the eye damage he sustained was either in the loops lifespan or a fixture from his time in the interim, the eye damage however looks like a cloudiness and loss of vision probably from aging and no real immediate recent damage is visible; so I assume this happened in the in between. Also vampire hunter makes a lot of sense but my dumb self thought he was straight up just supposed to be a grave digger in the first game lol.
the journal entry for the bodygaurds reads "Tusk pays them very well" (or something,) so I doubt they are [redacted for sopilers] like you said. overall good theory tho
I've always assumed Van Horsing says that about Tusk because he plans to kill Vlad, who would be related to her in some way. In my mind it was as simple as her also being a vampire (her tusks being a reference/being actual vampire teeths)
