You could save the file and take a print out outside. Or if you can't do that too, you need to visit the branch and ask them to activate net banking. Depending upon the branch, they will either give you an application form for this, or will ask you to submit a requisition letter.

Instead of trying to add in numerics at the end, try adding a few digits in the middle. For instance, for my name Jane Sheeba, I would try ja11neshe instead of janeshe11. Give this a try and let me know.
