When they were kids he unknowingly sealed her powers, which is the accident he wanted to get more control over himself to prevent. Then, in a situation that mirrors Leo's, he gave his body to Despair during the first Collapse to save White(mary). Despair (red eyes) shoots her but it's somewhat pointless iirc or just symbolic. She was already dead/dying and she refers to herself as a ghost because the seal that keeps her heart beating is weakening, and the thing that keeps Black from actively trying to reject Despair is a naive belief that he might be able to save her.
His eyes sees the truth. Like people names and can see through any possessions, illusions, or spells. It's very useful in this world since they have monsters that can hide, create barriers, and need to hide their ID. Like Old Ones in there cannot die unless you know their name which they never reveal.
