As John Spence wrote, this is a powerful and pivotal scene. The revelation would have destroyed anyone else. So much to untangle in such a short amount of time. And it is worth noting that Joe keeps his promise: the man from the coffee shop returns and makes it possible for a rightful romance to begin. When Death is sitting on the bench with his eyes filled with tears I always find that scene emotionally overwhelming.
What an absolute acting masterpiece by Claire Forlani. The slow transition from happiness, to fear, to hope. This should be a lesson to all of the current younger actors. If you can exhibit emotions of such different levels and colors at will, you are one heck of a performer.
playing that entire schene just be the eyes and very little mimics, full total - quite a high order for any artist. They nailed it, I was totally into it.

Right. By now she's figured out what he is, but doesn't want to call him that because that's what everyone else calls him, and death is just a label. She calls him Joe because that's who he is to her and it shows her respect for his personality rather than just his (unfortunate) job. This is such a great scene.

Saying they should have shown Death coming back for her when she's old. But that wouldn't work as well as you want. The significant part of this scene is when she mentions the first meeting at the coffee shop. It's when Death realizes what Bill was saying to him before is true. She doesn't really know him and it wouldn't be fair to take her on a whim of infatuation. Death realizes she had already fallen for the man whose body he is using. Everything that happened after was just a happenstance of life. It's why Death is so much in awe of life and why Bill's words "that's life" has so much meaning.
It is rare you see acting of this quality. You share her feelings in that moment. The film is long, even dull at times, but the quality of the actors lifts it far above the script. One hell of a tearjerker.
But, did Joe let her "see" who/what he really was? Or did she figure it out? It was so subtle that I wasn't really convinced one way or the other. I feel he looked at her a bit like he looked at the woman in the hospital. That "look into my eyes" kind of look. I love the subtle tremble and retraction she did, before calling him Joe. Such a killer performance and I don't think there could have been a better actress for that role. So captivating.
He tested her a little when she said, "I feel like I am lifting off." I bet he was deciding if he should take her soul then. Remember the hospital seen when he took the old Jamaican woman, he face showed same expression before he took her soul.
