Can't remember the exact wording of the rules at the time of the England vs Italy game, but it was along the lines of: there having to be players from both teams competing for ball for a ruck to form. The itailians game plan was all about not creating ruck, so therefore, there were no offside lines, allowing them to distrupt the English ball as much as possible. There's a clip somewhere of Robshaw asking the ref what they need to do to get a ruck formed in that situation.
