I actually got that from the movie :) If you watch closely you can see Sally flip the book around when she asks the aunts to bring Michael back from the dead when she throws the book on the table and says "Bring him back! I know you can". All the dark spells are in the back of the book.

Its a junk journal, there are lots of videos on how to make them. the images are from arty maze and others. if you do a search on youtube you can find lots of people that will show you how to make them. Nik the booksmith is good, so is Johanna. they are the ones i learn the most from.
