This Scene happened at Drum Island Arc the hometown of Chopper . This is where Roger and Whitebeard meet. Roger find Dr. Kureha to try to lenghten his Lifespan until his execution because Dr. Crocus is unable to do it. Remember, this is the Island where Blackbeard and his crew attacked when Ace is trailing him. He went to this island because he visited it with Whitebeard crew on the past . Just a theory .
He was sick from an incurable disease that was sapping his life little by little and was at the point left with little time left to live, before falling to such death, he told Rayleigh, Shanks, Buggy and all the other crew members to disperse to their separate ways not knowing what each other was up to. From there he turned himself in to the marines. His crew members found out by newspaper of the execution.

So I have a theory - “D” could possibly be from the old great kingdom like maybe they’re related to it’s king or something and the great kingdoms that was destroyed by combined might of 20 kingdoms (those kingdoms formed the world government) is it possible that the world government is just trying to keep the history of the great kingdom and “D” hidden because they might be related and if people found it they would turn against the government ?? Just goes to show how much roger respected whitebeard and held him in high regard when he told him the truth about the void history and the D clan,maybe roger believed whitebeard could finish the Job he couldn't because of his illness if whitebeard wanted to.
So I have a theory - “D” could possibly be from the old great kingdom like maybe they’re related to it’s king or something and the great kingdoms that was destroyed by combined might of 20 kingdoms (those kingdoms formed the world government) is it possible that the world government is just trying to keep the history of the great kingdom and “D” hidden because they might be related and if people found it they would turn against the government ??
P.s - I’m not super big on anime I just look up stuff when I’m curious about it
