    As an ex-canadian navy and army member, I know they completely revamped the dive course and took extreme measures after this happened.

      What about the maintenance diesel team not even filling trucks up before a trip because they are lazy?? what about the room full of scuba gear not getting taken care of for the 6-7 months of the year we dont use them??? just tell the truth... we are completly oblivious.

    I used to dive and I used to fly light planes. The lesson this video emphasized for me is : Don’t gloss over your checklist and never miss a step. The function of the checklist is to fill in those gaps in human memory. Procedure is important. An ounce of prevention is worth a pound of cure .

      @ David Zakrzwicz I got my private pilot's license through a Navy flying club when I was a civilian employee. My instructors had lots of hours in military flying and demanded i always use the checklists. They did too, leading by example. Cutting corners sooner or later ends badly.

      Dad iL was very eccentric - a life full of the usual excesses of people who were young in the 50s & 60s. Ex RAF but was a top dressing pilot and one of very few who lived until retirement in NZ. The reason? Never, never, ever missed his checks. (Long career so rare they made a doc about him)

    I actually worked for a former Canadian Navy diver, he was a Captain when he left the service. He was not involved in this incident but told me about it. They were doing a practice run for a big wig display the following day of all their different dive systems (open circuit and rebreathers for stealthily approaching "enemy" ships). Apparently they were delayed due to other preparations on the land, laying out equipment for a static display, earlier in the day for the practice dive so they kinda rushed things, skipping some checklists. The military is super anal retentive about checklists, but when you rush, mistakes are made and checks are skipped and it lead to this young mans death. The guy I worked for was stationed on the west coast when the incident happened so he got the after action report and lessons learned memos to use to train his staff/divers.

      I remember this EXTREMELY well, as my ex was in the FDU, (Fleet Diving Unit, Shearwater NS) a Clearance Diver and Stephane was a good friend of his. I thought he was also on that dive yet not mentioned (Tremblay). This incident was haunting. This is a re-enactment isn't it?

    Damn I've been binging your videos for a couple of days, and now I see you've covered this case: Stephan was my second cousin's father. Sadly I never met him (this happened a couple of months after my cousin and I were born) but really nice to have divers' opinions on what happened.

    I was recently (artificially) narc'd and the drunk analogy is so accurate and also very inaccurate. The feeling is similar however when you're drinking you KNOW that's why you're buzzed. I knew about nitrogen narcosis through guys like you, general knowledge of blood gas behaviors through academia, and was literally told what would happen by the expert who accompanied us in the decom chamber. I knew what to expect and thought I'd think "oh so this is being narc'd" once it hit. But despite knowing all that beforehand I still didn't get it when it was happening. Only after that did I understand why experienced divers have been narc'd and not realised it's time to leave.

      Wow! Thanks for sharing. I have wondered because I’ve read about or seen videos of divers who were narc’d continue on until their death instead of trying to get to the top. This explains why perfectly. A couple videos I’ve seen make a whole lot more sense now.

      @@BS-dq1kz it works differently on everyone. It's never the Dawn experience for anyone. Some people can't handle 20 feet. Some people can handle up to 500 to 700 feet without getting Narc'd

    When Woody was bragging about Gus' explanation I had to smile. I have been a subscriber for a long time and while not a diver I have always loved the water(to the point my mom could never get me out of the bathtub let alone the pool!). I appreciate your professionalism and knowledge(We have all noticed the quality improving constantly..and I want Woody's fancy shirt!), but honestly guys I think most of the subscribers would watch you two talk about the technical aspects of watching paint dry! Your personalities are both so fun which really makes the videos for me, as well as your obvious expertise.

      Thanks. We are always trying to improve with these videos and frankly with our diving and diving knowledge. For example I just listened to Simon Mitchell’s explanation of what happened to David Shaw. So when I said “he was alone” in fact he was alone at that depth and the scientific explanation of what happened was fascinating and I want our follow up video to include Simon’s explanation.

    I remember hearing an old master chief telling this story on the mess decks one night on my first deployment. He wasn’t in the navy at the time, but his first lcpo was there when this happened. He used the story to warn the junior sailors who were talking about wanting to be divers, a cautionary tale. It’s a sad story, but kinda cool that I found it here.

    I'm a 15 year old kid whose been around water his whole life. I've swam and snorkeled for over 3/4 of my life, but have never dove. These videos make me want to dive! You guys do a great job of explaining what's happening as well as what should happen and what could happen. Thanks for making these videos, congrats on 10k and keep up the amazing work!

      Love this, I love snorkelling myself. Scuba is a fin too far for me. I'll stay close to the surfaceI I think.
      Each time you go into the water, just before you get in, think of Gus n Woody, and triple check plse.

    I’m so confused. How come someone didn’t just immediately jump in and grab him? They knew he was there 10 seconds ago. Couldn’t someone have just jumped in and immediately have grabbed him??? It seems like they wait forever to send in the rescue guy

      He could have been suffering from oxygen deficiency and not fully inflated his bcd, could possibly even have deflated his bcd in confusion and could have sank pretty quickly, even if you were in the water 10 seconds after you noticed the diver was not on the surface it could be difficult to catch him, we also don't know what the visibility was like. You will disappear in a split second in poor viz, Literally take your eye off your buddy for a moment, turn back and they are gone.

    I watch the videos for YOUR commentary. Anyone who wants to watch the original without it being stopped should watch the original. To be honest, I don't really find the original videos to be that interesting without the commentary.

    This channel has helped me a lot. I’m no diver. Not to much into it either. But these two make such good content im addicted ! And have some weird sentimental grasp on these two. There good energy and vibes are getting me through some really bad health problems, so that must be why, thank you so much for the content.     Hey guys. I'm a big fan and love watching your videos. My dad use to be a diver, with thousands upon thousands of hours of diving. He did everything from diamond diving off the Skeleton Coast, diving shipwrecked for the museum, to rescue diving. So know that I have a lot of respect for you guys.
    Now with this video. If I may make a recommendation. When you guys do stop the video, which is necessary, I would recommend rewinding just a little bit before playing again. This may lead to less confusion. I had to rewatch parts of the video again, to make more sense of it.
    This is not a critique but an observation that may help with future videos.

    My dad is a long-time diver and even worked for about a decade on the dive team for our PD. I could never get certified cause I don’t do well the instant my face and mouth get covered. I panic and I knew it was too risky to even consider doing it because I’m also terrified of open water of all kinds so those two things coupled together didn’t seem worth trying to fight against every time I went under. I knew I’d eventually lose and potentially cost lives other than my own. So I still worry every time I know my dad is going down.
    I appreciate you guys so much for going out of your way to encourage safety and caution at ALL times in every situation. You guys have a respect for the water and the deadly potential that exists around every turn and at every minute and that is, in my opinion, the first thing any diver must know and respect.
    I’ve been binging your videos. Thank you so much for the content! Stay safe! ❤️❤️❤️

    Woody, as a professionell Pilot for over 45 years I love how extremely observant you are truly and how serious the two of you go about diving!
    As a not yet diver, I love your highly educational show: Learn from the best best and never quit learning was my philosophy in flying and obviously it is the very same here. Learning from the best is what my butt saved a few times. I lost many good pilot friends over the years and I contribute my survival to the fact of using check lists, knowing the systems insight out plus 80% of a successful flight/dive is proper planning including options a/b & c and knowing the limitations of your plane/gear, but most of all properly judging your own limitations!

    Former US Army here, there’s an important bit at the beginning you didn’t notice/decide to comment on. They said an Admiral was coming and they were prepping for “the usual dog and pony show.”
    Basically, everyone in their unit is trying to make everything LOOK as good as possible for the Admiral. That means everyone was trying to make their job area look perfect, and DEFINITELY not paying as much attention as usual to a routine dive.

      Yes, that is so true. When I was stationed in Korea, we had a Division Change of Command the week before we went to FOAL EAGLE Division FTX, and I knew something was going to go very wrong. I could just feel it, because the soldiers weren’t mentally prepared to go to the field. And we lost one of our men at the end of the FTX when an M1 Abrams tank was passing by our soldier who was on the asphalt roller. The road gave way beneath the roller, and the roller toppled over on top of my soldier and smashed him to pieces in the ditch by the Imjin River. It was horrific. Eighteen years old and dead because of the dog and pony shows that literally nobody except the commanders care about. The whole FTX was a giant clusterfuck because almost nobody’s head was in it, and that’s what led to that dreadful mistake.

      Former US Navy in my case, and have been a "dog and pony show" presenter. I can picture a specialized unit (like a dive unit) wanting to demonstrate their range of mission capabilities. This would naturally lead to demoing a wider than normal range of equipment on a single dive.
      Being an airborne unit, we did demo jumps, but thankfully only one type of equipment (static line). Another unit did HALO, which was outside our mission.
      It blows my mind, though, to have a standby diver without his weight belt on and having checked his tank pressure.

    These guys got me interested in the logistics and thrill of diving. I don’t dive myself yet - mostly due to the fact I consider myself too young to commit to anything - but there is something marvellous about learning more about recreational, professional, and every other sort of diving and its implications and inherent dangers, alongside its many upsides. For that learning, I come here, to hear both of you guys’ commentaries. Both Woody and Gus are excellent in their fields and extremely insightful. You guys should never have to apologise for teaching us something here and there whenever you happen to pause the video. Keep it up guys… this stuff is phenomenal!

      I love it because I'm an Advanced Recreational Trimix Instructor but Woody still acts surprised when I explain something related to how gases behave underwater...like "Wow, very good Gus, you are such a great student", I'm like "Woody, I'm a Tec diving instructor bro" :D
      He definitely knows more than me about that stuff of course, and he's a much better instructor!

      @@DIVETALK Being able to explain concepts like hypoxia/hypercapnia and the function of a rebreather in a way that's palatable to non-divers is usually more impressive than just understanding them. It shows that you've really mastered the concept instead of just memorizing the information.

    I don’t know anything about rebreathers, but I am an icu nurse, it does look at this point like hypoxia, however increased CO2 will cause a change in mental status and will cause you to pass out we do intubate patients with high CO2 to protect airway.

      I’m an icu nurse as well, amd was thinking the same thing. COPD (similar to emphysema) can cause problems if a pt is given too much oxygen.

    Very interesting though tragic video. When you mentioned being careful not to get complacent with safety checks, it reminded me of a story about the Japanese railroad system and how they have this method of carefully "pointing and calling out" every safety step and it reduces errors by like 80% or some crazy amount...safety first!

    I'm only 8 minutes in, but I'm dying to know when, and if, they realize they're reacting to a reenactment video from the story the survivor is recounting... Not that listening to you react to the reenactment isn't packed with amazing information!

      Came here looking to see if anyone else noticed this too. The first major clue this is the case is when in the video he's holding on to the boat before disappearing, but doesn't mean that is actually what happened, but the commentary made it to be that was a fact and a 'major' part of their analysis.

    Indeed, those checklists exist for a reason, the most knowledgeable and experienced experts are still human. Watch some of the plane crash investigations - the number of incidents that would've been prevented had some 4000+ hour pilot not missed one simple thing because instead of using the list, they just did it in their head.

    I’m hard of hearing, specifically with an audio processing disorder (hear the sounds but brain doesn’t recognize the pattern of sounds as specific words). Stopping the video makes it so I can actually hear you. When two different sounds happen at once I get confused. And also, you plus the viewers would miss what happens while you’re talking. It doesn’t make sense for anyone to not pause the video if the point is to comment on it, imo. Thank you for stopping the videos to speak

So many people think they just need to talk louder at me, and don't realize how hard it is for me to understand them when there are multiple conversations taking place around me.     I can’t believe they skipped so many checks. I was a Jumpmaster in the Army, and I’d never skip an inspection, skimp on an inspection, or fail to keep checking the gear of my soldiers and myself. I was terrified someone would die if I screwed up, and I didn’t want to also get in trouble with the Army for not doing the right thing. They could really bust you down for this sort of thing. You could lose your rank and even your freedom if found negligent.

    Unfortunate we would never know who/what this “foe” was from this incident. But “fatigue” is unspeakable when you are in the military. No military will ever admit that fatigue is the cause of death in training. This indeed is a formidable “foe”, deep-rooted in military mindset forever. I recall blacking out once while on a battalion exercise. And many of my company mates did as well. Those who did not, told us that even they themselves were just too tired to think, or eat. And this was ON LAND!

      Yeah I’m not getting this at all. How does anyone drown at 15 feet with a group of divers aware and looking for them, let alone navy seals? It doesn’t make any sense. It’s 15 feet. That’s only 3 feet more than the deep end of most swimming pools.

    the only reason i can come up with where the weight belt would be thrown in the water is to mark the bottom, other then that i don't know. My ultimate question is why didn't a couple guys jump into the water right away (even without the dive gear) grab the dude and swim him back up?, seems like alot of civilian type disorientation.

      Well, on the video it seems like they helped the other two divers get on the Zodiac while the other diver was sinking to the bottom, so by the time they realized he was gone maybe a minute or two had gone by, the guy is also wearing all black and not easy to spot, so they could've jumped in and start freediving hoping for the best but I don't think it was a situation where they can see the guy but decided not to jump in anyway.

      @@DIVETALK Not to mention just free diving in trying to help a diver would be against most rescue diver principles. The best a free diver could hope for is quickly finding a functional inflator to bring the distressed diver back to the surface. I'm not sure I'd risk that unless I was positive the distressed diver wasn't a panic risk and I could visually see them before jumping in. If I had a safety/rescue diver suited up ready to go, I'm sending them in and either calling emergency services or aiding the rescue from the boat as someone else calls it in.

      They can't, but they can only theorise about what they actually see.. Based on what they saw in the video thats what they think. If the reconstruction is wrong.. well then there might be other reasons. Still a valuable lesson.

    Just found this channel; love the videos!
    As an anesthesiologist (and diver, open-water certified), I had to give a little extra physiology lesson on drive to breathe. You guys are absolutely right that CO2 buildup (and change in blood pH from that) is the usual drive to breathe. However, there is something called hypoxic drive as well. We have peripheral chemoreceptors that respond to low PaO2 (has to be very low, around 60mmHg before this happens, hence why it isn’t a factor in usual breathing). So, in cases where someone can ventilate (eliminate CO2) but not oxygenate (like a low FiO2 situation), they can develop this hypoxic drive to breathe and get into respiratory distress (not to mention the other physiologic effects of hypoxemia, leading to tissue hypoxia).

    It's pretty obvious the video was a recreation of the events.
    Frankly, I would put little faith in how accurate the events were portrayed such as rope grabs and what was thrown except that which the host mentions.

    Just being constructive, its hard to focus on the video and your voices at the same time. I'd like if you both could stop the video immediately, so you'd never be talking over the video.

    Love hearing the science behind diving! I’m not a diver but still find it so interesting. Love the way you both break down explanations and science for these videos!
