Since we see that Angela had 10 almost full term pregnancies that she lost (this is why she's so messed up btw, she never confronted the grief of those 10 miscarriages, just repressed it) we can assume they've been trying to have a baby for at least 6-7 years by that point. So a time frame of 1948-1955, well before the 60s when sstudies revealed that smoking affects pregnancy. This is why Clay is seen just staring as Angela tells him about her pregnancy with him, he knows that smoking, drinking and her generally exciting lifestyle (horseback riding, Rollercoaster riding, trampoline jumping) contributed to his unborn siblings' death and he easily could have been one of them.
If you go by the shows timeline that would have been around the 1930’s where that information was not well known yet. What may seem obvious now was not back then.

doctors in the 30s - 50s were advertising tabacco like it wasn't harmful, some (for a paid fee) even encouraged it, so, many took it on i think some knew it had effects, but just like the government encouraging dairy consumption because it produced too much and wanted its citizens to consume it all, tobacco was profitable and getting everyone addicted is also profitable.

1:30 and 1:43 I like the subtle detail of Arthur slowly drifting away from Angela with each photo as she repeatedly continues the bad habits that cause her to miscarry. Knowing that he believes more in logic than blind faith, I get the feeling that he initially tried to warn his wife about what the actual problems were, but just like Clay she was so stubborn in her own beliefs that Arthur eventually gave up on trying to convince her.
Can you imagine how much of a jerk Clay would have been if none of his brothers and sisters were stillborn? It's quite clear that in his mind, he MUST be the center of attention. Why else would he also antagonize his father when he's told that he's not even worth being hit and came to associate abuse with worth and affection?
I blame the mom for that. Clay was smothered all his childhood from his mom and when she passed, he obviously wanted to have some sort of relationship with his dad. If you see the marriage episode, Clay somehow turned out quite a smart intelligent young man until Bloberta got him into alcohol

American here! And I watched every episode back when they first aired... so this was literally the most traumatizing thing on the air until Rick and Morty came along. Just so everyone over there knows, there were three episodes from the first season that were not initially aired because of the censors. Public outcry from the fans forced them to air them eventually, and boy did we get what we ask for! 😎
Angela maybe a sweet, loving mother to clay. But she is SO STUPID to think that smoking, drinking, horse back riding, riding roller coasters, and jumping on a trampoline would NOT hurt her babies.
Now i wish they have done this with Draco Malfoy and Narcissa. Imagine her telling her son he is her ONLY precious EVER. Even Lucius evil deed at this moment, would sound like puppies compare to the whole truth. One thing is sure, the show is promisingly DARK.

My favorite episode was when Moral got caught going number three in the school bathroom and went on to impregnate half the women in his neighborhood.
