@Derek Sumner It's a storyline brought up from a Bleach novel that was published in japan with Hisagi as main character. There's a new antagonist who apparently managed to replicate hogyokus and wants to get involved with making a new Soul King. Hogyoku will work for anyone as long as there's intent bihind it, Espadas themselves were previosly Vasto Lorde hollows who became Arrancarrs though Aizen's Hogyoku in first place. I dropped the game not too far ahead unfortunately because I don't have enough time but you should be able to find the storyline in both the game and in fan translations of the novel "Can't Fear Your Own World". There's quite a few new characters in it. (If I recall correctly it happens after the Quincy War arc but before the 10 year skip at the end of Bleach manga)

No, this is their reborn forms made by merging them with (pseudo) Hogyoku (filling their holes). The proper second form is still unique to Ulquiorra.
