I guess that maybe because Sam it's more calm. Looking suddenly at a teenager or an old man acting totally out from their age must be "that definitely is my brother Dean". But When Sam swap body with that teenager he did his best to blend around, Dean wouldn't be capable even if he tried!

Ironically for people in their line of work and with their experience. Figuring out how to never get old is probably pretty simple for dean. He just never saw the need until he learned about acid reflux lol..
