What if limit is one bounce per over and bowler already bowled one bouncer. Now next ball bounces over shoulder is no ball that is clear. But what if bowl bounces over the head and no contact made by batsman. In such case it is wide bowl or no ball ?

Where the ball pitches doesn’t matter as far as the off side is concerned. If by “pitching” you meant impact, then it is in fact in line with the stumps.
