      She’s precise. Nurse here, I’ve heard numbers far above 10 more times than I can remember. Fentanyl for an 8.5 though?

      @@kimkirby7602 It depends on the agency, Our agency only has Fentanyl that replaced Morphine. We will soon be getting Ketamine as well but the protocol for that will be strict for only serious injuries such as your open compound fractures etc.

      SBAR. situation, why you were called. Background. PERTINENT information. ie the blood thinners, assessment/treatment, and their response. Trauma report may be different but hand off report at hospital and narrative in PCR should be pretty similar. Just different lengths .

    I'm currently in EMT school so it's awesome to see the SAMPLE and OPQRST assessment in action along with lifting and moving, palpating, vitals, and gentle and caring communication with patients. This is all stuff I read and hear in class but seeing it for real is amazing. I need to watch this video over and over to learn how to give the perfect radio patch! That's one of the best ones I've seen! Incredible video. Keep em coming 🙂 This is my dream department ❤

    I really love your show! I believe you are educating people as well as just showing what you do and that is so important. Thank you for everything you do for people on a daily basis. I appreciate you talking about the difference in Fentanyl given legally opposed to that people get on the streets! I use it in patch form and have seen so many reactions in people.

    Watching SWMFD from overseas, I can't really find the right words to say how cool it is! The mindset, the gear, appartures, dispatch, everything is organized and ready to go....
    Beeing a Paramedic with a big fire department, 16 years of experience, we have never even been close to that, what you do on a daily basis! Just love the work and effort all of you putting in the daily work 👍👨‍🚒👩‍⚕️👮

    I see blood draw items. Do all your hospitals except blood draws. I work in a system where community hospitals except along with one city, then the other city doesn't.
    I love these episodes and all TH-cam content your department creates. It's so awesome to have all professional content, no relationships like in TV drama, real or fake.

    Cool, safe landing but an exciting call to say the least. Getting to see how they coordinate how they are going to respond given this situation, so they are both on the same page was educational. I think SMFR Dispatchers are the best. It seems like they transmit accurate information and retransmit/relay it the same way to those who need to hear it. I have listened to other dispatchers from other areas, and they seldom or never retransmit incoming information from other units to say everyone is in the loop. I can relate to this sick woman's dilemma, drove myself to ER hoping for an antiemetic, but after 12-hours of emesis all the n/v ceased. Good med review by Paramedic on medical Fentanyl usage to reduce pt. anxiety. Excellent people skills. On that fall pt. Dirty Harry would have said I'll take them off. Very professional in every way. Always talking explaining what is going on and assessing pt throughout transport. SMFR Medics are the best. Thumbs up. Great video coverage too.

    I really enjoyed the three episodes so far and I'm excited for more in the future! I'm working towards becoming a FF/EMT and these videos are nice to watch to be able to better prepare for it. Thank you for creating them and thanks to all of the first responders in the video who are making peoples' lives better!

      The dash cam we're using for these episodes is an Insta360 X3. We edit the viewpoints after the 360º video has been captured.

    Ha ha, such a classic situation with the first patient...
    The medic was half-way towards a diagnosis of a heart attack (ordering the 12-lead and trying to locate the pain), when the patient mentiones a fracking gallstone for the first time after propably about 15 minutes of being on scene with her. Just goes to show how important a good and thorough anamnesis is, people who aren't medically trained (and aren't expected to be!) often don't realize what part of their medical history is more relevant than others.
    Question: I assume you use the xABCDE algorithm for patient survey - do you also use the SAMPLE for further assessment or do you have other methods?

    Good content but it’s curious why the account doesn’t provide any comments or answer any questions in the comments. Such as why give pain meds to a patient with a pains scale of 5?

      I like your question. I have a lot of questions about the treatment of the fall victim... she's bravo condition at best and he gives her a full trauma workup. There is no need to cut her pants off. The c-collar is not indicated in any protocol I've ever read for this type of situation... She's not complaining of neck/back pain, no abnormalities were noted during physical exam, she's GCS 15, no distracting injuries, etc. As for the pain medication... I'm okay with it but if you're going to give it why not give enough to take the pain away? There is zero benefit to keeping a patient in a state of pain. Lastly, just give Zofran with the fentanyl automatically to prevent nausea rather than playing catch up by waiting until she is nauseous.

      Our Public Information Team is very active across SMFR's social media platforms, including TH-cam and we're happy to answer questions. Some of them require asking subject matter experts and take a while longer to get answers. Pain management is a decision that is made between the Paramedic and patient based on the situation. The patient experienced enough pain to cry out while being moved and their pain scale was 5 while the ambulance was stationary, so the decision was made to try and alleviate the pain knowing that road bumps and more movement was coming at the hospital.

      @@SouthMetroFireRescuePIO Thanks for the response. Every department operates under their own protocols based on local medical control. With a patient slipping from a standing position and describing the pain as numb and maybe a 5 we probably wouldn’t have given pains meds especially in an elderly patient. Have no issues with patient care if local protocols were followed.

      Thank you! We're using the Insta360 X3 for the rotating video angles. A Public Information Officer does all of the video camera operation and powers them on as soon as possible when call is dispatched.

      Have you done any ride alongs? If you're able to sign up for ride alongs with fire departments in your area, that would be a great way to see the medical part in person and find out if it's right for you. Spending a few hours or a day with the crew will lead to some great conversations and lots of tips to help you as an individual since everyone is different.

      RED1 & RED2 are cross-crewed by Tower-35 so two firefighters on each and RED3 is cross-crewed by Engine-44 so all 4, the response plans for an Air-Alert-2 (standbys) is - RED1, RED2, RED3, 1 Medic, 1 BC, 1 Safety
      For Air-Alert-3 (aircraft crash) is - RED1, RED2, RED3, 2 Engines, 1 Aerial, 2 Medics, 2 BCs, 1 Safety, 1 MED and on-call PIO.

      Tower 35 cross-staffs Red 1 and Red 2 so each of those units respond with 2 personnel. Engine 44 cross-staffs Red 3 and their crew of 4 usually all travel in the ARFF unit.

      Um, who knows what “policy” is but don’t trust a monitors initial BP… manual first, it’s just good practice. Especially when your dealing with giving medications.

      From what I see their monitors don't have BP cuffs on them at all. It's a decently expensive add-on to have NIBP added to any of the commonly used EMS monitors. Plus the monitors are not the most accurate, so with the cost and patient benefit combined I believe SMFR sticks to manual BP's only.

      SMFR doesn't have BP enabled monitors because the research our EMS Division found determined that they're unreliable in dynamic environments. New monitor upgrades will be coming in the next year which may include the newest BP technology, which was recently found to be very accurate during testing at our training center.

    Does everyone get a rotation on the medic? So if someone wants to work on the Tower do they have to work the ambulance at some point?

      South Metro determines riding positions based on seniority, but all firefighters during their probationary period will gain medic unit experience. Once firefighters are off probation they can bid for a specific seat on an apparatus on the shift of their choosing. There are also roving firefighters who work at various stations and on various apparatus as needed.

    My aunt told me I should go into the medical field....she has no idea that needles & or the sight of blood make me queasy & woozy. I'm finally able to get thru all the needle stuff, as long as I don't see it coming lol. I still can't stand seeing blood, including my own. Even more embarrassing, I'm a "sympathy puker"...if I see it or hear it's going to cause a chain reaction. My ex husband was barking out questions one night about what the sounds were coming from the bedroom. My cat ralphed on my nightstand & I ralphed too trying to clean it up lol! At least I made it to the bathroom lol. Unfortunately, I repeated that game winner with a 2nd mad dash to the powder closet lol. I yelled at him that the cat threw up & it made me throw up...he said something stupid so I called him a f'ing a**hole under my breath. Of course he heard that even though he was deaf to every other comment I make lol. Bless you folks who work in the field of medicine.

    I see Americans are still going crazy with C-collars in any traumatic event.
    In Europe we don't do that anymore. Either you completely immobilize a patient or you don't do it at all, as a C-collar only immobilizes parts of the cervical spine.
    Also if we do decide to immobilize the patient we do it before moving the patient on the stretcher or in the ambulance.

      We are slowly moving away from it as well, It all falls back on our medical directors who make the final calls, Ours recently got laxed and we only board and collar PT with severe MOI like a high speed auto accident or with obvious injuries like a femur fracture etc.

      SMFR policy allows providers to use their discretion when wearing masks during patient contact. That was decision was based on CDC recommendations and low Covid-19 positivity rates in our area. Hospitals in our area have maintained the mandatory masking policy for patients and providers who enter their facilities.

    Question for anybody with EMS experience. I’m pretty much 90% switching careers and pursuing a career into EMS but I don’t necessarily want to be a firefighter but I notice a lot of medics are firefighters/Medics. My question is it required to be a firefighter to be a medic/EMT?

      Great question! Many cities, counties and private companies have EMS careers that do not involve firefighting. For example, the City of Aurora and City of Denver both border South Metro and use ambulance providers that are separate from their fire departments. New York City among other places have EMS under the fire department organization but their EMTs and Paramedics aren't required to be firefighters. So, you've got many options if you're interested in a career change!

      Centennial Airport does not meet FAA Part 139 criteria with scheduled flights over 30 passengers so ARFF is not required to be on site.

    I am curious what SMFR and Colorado state protocols are. From my experience and according to my local protocol, that fall victim is over-treated. Cutting the pants is unnecessary. A c-collar is unnecessary. And waiting to give an anti-emetic until the patient is nauseous is waiting too long with fentanyl administration.

      LOL, Then your experience sucks and you have literally no idea what you are talking about. Cutting the pants is extremely necessary, how are you going to be able to check for swelling or bruising with her pants covering the injury? C-Collar is just a precaution more so for her age than anything, and it is very normal to wait to give Zofran after pushing a narcotic, first fentanyl does NOT get people as sick as morphine does and secondly if they do start to feel sick all you have to do is push the zofran and normally within seconds they are already feeling better. Zofran is extremely fast acting.
      I am curious what your experience is because from the looks of it I feel like you got it from watching Greys Anatomy.

    14:15 "your heart´s lookin´good", and what you can see at the display of the really shitty LP15 are the typical artifacts in ECG cause this device is really terrible in writing good quality ECG. I hate this device and I am glad that we switched from LP15 to C3 Touch

      SMFR doesn't carry blood but an IV blood pump is often used during traumas so the hospital staff can use them.

      Coveralls are a personal protective gear option at South Metro that individuals chose to wear during medical calls. That practice started during Covid-19 when disposable gowns were unattainable and the coveralls would be washed after each call.

      Mechanical fall implies that an external force caused the patient to fall and there's no underlying pathology of concern and/or the patients did not pass out first.

      Med 2 was placed in service on 2/20/23. It is staffed by an EMS Captain and housed at Station 37. It responds to the same type of incidents that Med 1 does, but usually covers the western half of the district.
