The bladder is where you put your clean plain water and the dirty water goes in outside the clean water bladder. All in one tank. The formula tank is on the side as you figured out already. This machine is indeed from 2006.
My grandma had a model similar to this called the 9200, it was about the same thing except it was blue. That was a very good machine but we ended up giving it to Goodwill because we had bought one of the newer Hoover carpet cleaners, which I am not as fond of.
I used to have have a Bissell proheat carpet cleaner in a blue color years ago when I was a kid. It quit working properly, I’m guessing it lost suction somehow.
A lot of these were sold and a lot of these were thrown away. I see these all the times in vacuum stores. If they said the spray is low it’s either the belt is worn out, the pump is bad or the heater is clogged.
I can't look at the model number as it is my parents machine. When I was in College we got a machine in Summer of 2003 that looks like that machine. I assume it still runs as I only use it and it hasn't been used in 15 years since I moved out. There should be a sprayer like attachment to but the Scotch Gurard bottle on so you can spray it onto floor or furniture. I've head Bissell pumps are famous for failing.

I have the newer version of this and it’s a decent cleaner. Mine doesn’t have an actual heater, it just redirects the heat from the motor.

YES, it is probably 16 years old, those were made in the early 2000's
