Yes, there has been at least one instance in the Indian Premier League (IPL) where two teams ended up with the same net run rate (NRR) at the end of the league stage. This happened in the 2010 IPL season.
In the 2010 IPL, the Kolkata Knight Riders (KKR) and the Rajasthan Royals (RR) both finished the league stage with an identical NRR of -0.341. However, despite having the same NRR, KKR finished sixth in the standings while RR finished seventh. This was due to the number of wins, with KKR having won one more match than RR.
Instances like this highlight the complexity and occasional quirks of the NRR system, which can sometimes lead to very close calculations and unusual situations.

This would never happen. Even if this happens. Best possible way to decide is to calculate the NRR in 2 h2h matches alone.
