Great question. I think this is up to you and where you draw the line. Why may be helpful is having an example of what just one change looks like, like making the background red instead of green. Then you can define what counts as two changes, like adding in a dog toy or moving the pet over the right. Give specific examples. And no normally clients don’t ask to make such huge changes. Just minor ones like making the tongue smaller or taking out the whiskers.
