Excellent video and presentation. Basics of how to evaluate breast on mammography is succinctly presented. But thing that you missed is classification of breast density as per ACR guidelines and I expected it to be covered in basic lecture. Some of the things regarding how to examine were new to me.
Thank you for appreciating the video, Dr Mundhada. Your insightful review shall definitely help me ameliorate future videos. I have done a short video which explains classification of breast density as per ACR guidelines sometime ago. The link to watch that video shall be shared shortly.
Meri right side breast mein lump tha 5 years earlier uski surgery hui thi uske baad se mujhe right side breast mein pain, itching, numbness rehne lgi. Problem find out krne ke liye humne ab mammography krayi mammography both sides ki hui and after that dr. ne right side breast ka ultrasound bhi kiya unhone btaya ki ultrasound mein koi problem nhi h sab normal h but jab unhone mujhe report di usmein mammography mein sab normal likha hua h usg mein fibroadenosis in one side and impression mein fibroadenosis in bilateral breast likha hua h. Mam how is it possible jab ultrasound right side ka hua toh wo dr. Kaise keh sakte h fibroadenosis left side mein bhi h maine unse yhi baat puchi toh wo kehte h aapke breast x-ray se aise lag rha h but unhone mammography mein sab normal likh rkha h and agar fibroadenosis ki wajah se pain hoga toh woh both sides hona chahiye but mujhe only right side mein problem h. please suggest me something what I have to do now. Am i right or wrong?
you mentioned about how to measure the size of breast using paper and ruler(scale). How it helps in interpretation of mammography ? Also in two views (MLO and CC of same breast) difference more than 1 cm , how it is to be interpreted for pathology and how it should be reported in a report ?
Hello Dr Mundhada. Thank you for the query. Accuracy of mammographic interpretation depends on adequate technique and positioning of breasts. PNL measurement for CC view within 1 cm of MLO view suggests satisfactory positioning, indicating that the breast is pulled as far forward as possible during positioning. It also confirms visualisation of fibroglandular tissue that extends posteriorly. A difference of more than 1 cm in CC and MLO views of the same breast indicates improper positioning resulting in inadequate imaging of the breast tissue which extends posteriorly. This hampers the diagnostic accuracy of the test by missing out on pathologies, if any in the that area of the breast. It also poses a problem in interpretation of follow up cases which show developing pathologies in that area on future mammograms. A repeat single view should be performed with proper positioning and adequate forward pull-out of the breast tissue. Reporting should commence once films are obtained ensuring correct positioning and technique.
Very nicely explained Mam, Thankyou So much.

Why breast compression reduces thickness of breast ?
It will spread out the overlapping different tissue, helping delineating them in more details, thus exposing any possible lesion.
👍Thanks
