  • Join me and my husband, Mark, for another day in the life of a doctor. Today we're seeing patients in the hospital. You'll learn all about liver cirrhosis, the fluid that builds up in the abdomen and the procedure to remove it. Plus learn about the many conditions that can cause high blood pressure and hypertensive emergencies!

    One of my university professors (I finished 10 years ago) was from the Sault.
    I also remember when my grandfather had cancer (he passed away over 19 years ago), he had fluid on his belly too.

    Oh, the best husband and wife doctors in the world. Thank you for the video. Specially the liver stuff. Oh my gosh I hope he’s feeling better. That happened to me when I perforated my colon they sent me to interventional radiology within two days and then they drained me and I felt so much better. Thank you both of you for saving lives.     it's crazy that I started watching these videos years ago, just fascinated at the medical info. now I'm in med school and could follow along in depth and understand everything! Also, what great timing, I'm in the thick of my GI unit (learning liver!) and finished learning hypertension in my cardio block weeks ago!

    I love that you mentioned prescribing the park passes! I live in Canada and am extremely interested as someone who suffers from mental illnesses. I was wondering if you knew if any doctor(ie. GP) would be able to prescribe the park passes or just emerg doctors?

    I've never heard of a "Hospitalist" doctor, so I googled it, and it is a new-ish profession. The first reference to this job was 1996.

    Hello Dr. Siobhan, This is an important video of you about treating a patient with Liver Hypertentive Disease. Actually my girlfriend had this sickness of hypertension and she had high blood pressure. Finally unfortunately she died by the effect of hypertension which caused her heart stop functioning. I was sad about her Siobhan

    I have 3 sleep disorders that are treated with medication. Onset insomnia, slow wave sleep deprivation and restless leg syndrome. I had difficulty falling asleep and didn’t have stage three or four sleep which are the restorative stages and because of the lack of deep sleep I was constantly awoken by the restless legs jerking me awake

    Love this video! Great teaching. I love seeing the diagnostic process. Also, it's great to see a younger doctor using dictation. The older doctors I teach are sometimes a bit embarrassed that they can't type faster. I tell them don't be. You didn't go to school to be a dictatypist, just think of it as just another clinical tool.

    I was 200/90-100 hypertension stage 2 metoprolol 50mg only 17 was 16 when i got hypertension and panic attacks they last 3 hours long i passed/fainted once which was on camera at eneegency wait room and i woke up at disabled to kove normal and coukdnt walk at all on my ny own i almost fell so i told the nurse i am to weak to walk.😊

    Siobhan, I was having high blood pressure at 15. Thank goodness they did all the tests that you mentioned. I was diagnosed with Chronic Kidney Disease at 15. I have been living with the disease for 5 years.

      I can totally relate! I'm 23 having the same problems now... i too have possible CKD but waiting on biopsy results to confirm... i hope yours is under control Lilla :)

      I feel for you. I was diagnosed with CKD at 27, but it was suspected when I was 22. I've now been living with it for 26 years and have had a successful (live donor) transplant. There is light at the end of the tunnel, even though it may not feel like it at times. Ruth

      The kidneys regulate blood pressure so CKD can lead to blood pressure changes or fluid retention

    What exactly is a hospitalist, and how is it different from internal medicine? I've never heard that term before (I'm in the U.S. so I'm not sure if we even use it here).

      I did some Googling, and it seems the US also has hospitalists! The role seems to have changed through the years, and it seems now it's more common for hospitalists to work more on patient management than an internal medical doctor (in terms of making sure the best possible treatment plan is coordinated between a patient's care team, plans after discharge and for rehabilitation, and treating patients in the hospital for example), while it seems that in internal medicine the goal is to treat immediate issues. To my knowledge, if you're a physician in internal medicine you don't work exclusively in a hospital setting, you also likely work in clinical settings too while hospitalists work solely in the hospital! Those are the major differences I can tell, there's probably more.

    I had a sleep study done because I would wake up constantly due to my mouth being wide open which dried out my throat and made me cough often. Study showed I had 69 episodes per hour! They Rx me a chin strap to keep my mouth shut and an apnea machine to ensure I continue breathing. Since Sept my readings show an avg 1.5 episodes per night - a huge difference. Those episodes also resulted in my having hypertension so I'm on an ACE Inhibitor now. I never thought how sleep can affect your BP. At least I don't wake up exhausted anymore

      Day to day it often feels quite equal, but many of the diseases I treat are more common in women, so I bet if I counted, there would be a skew to females!

    Have either of you ever been to St Boniface Hospital in Winnipeg? Either Mark has a twin, or he was helping my friend last evening in the ER. I didn't realize until after the fact, was a little preoccupied.

    I'm really curious about how the finance/billing of this all works, whenever you travel to Sault Ste. Marie to help with the doctor shortage. Are you directly billing OHIP for each patient you see? Or are you paid a flat rate for your time by the local hospital? Or some combination? And is this the same or different from how you're paid at home by your home hospital (if you're working at a home hospital right now). Are you employees of anything or are you self-employed?
