I see comments saying that they're fighting for control but here's another theory based on what Christian (DPR Ian) said about the lore, but also about the real state of his mind now as he's been struggling with the manic highs and depressive lows of bipolar - Mito is the protector, born from his traumas, that he keeps hidden, even from Ian himself. Ian is the balanced "host", the man we see (in the Mito movie, he's the one being kidnapped by Mito, not fully knowing what's going on), a normal man trying to deal with his overwhelming emotions, while Mito keeps the most traumatic events from the past hidden from him.
Now Mr. Insanity is the god of the created universe. He created Mito - he was one of his first creations. And Mito despises him for it and wants to destroy him and the creations Insanity loves. As Insanity represents the manic episodes state, he's invincible, hyper, reckless, thinking he can win with anything. But here he seems scared of Mito as his creation has become stronger than himself. At the same time, the door he wants to unlock with the green screen guy may hold all the memories of past trauma that Mito has been guarding. That could lead to destroying Ian, but also Mito, helping Insanity, well, make everything in his image again - insane.
There is one more character that you'll see if you react to the older videos - Scaredy Cat. He's the personification of Ian's anxiety, stress and nerves, who became this way because of Mito and the fear of what the dark one can do to them and the others around. Because Mito can take control over their body, over Ian's and Scaredy Cat's. The only one he can't control is the clown god. Mito retains all memories but Ian doesn't remember what his body does when Mito is in control (same as Christian doesn't remember what happened after a depressive low which can last for days or weeks even) - so at the beginning of the older videos we see pictures with a shadowy figure in them - Mito. Ian revisits the places, searching for what he should know and recognise. And from that point we're going to Mito pt 1 where we see how Mito is a fallen angel, cast away and hurting, seeking revenge. This part is still a little unclear because Mito is seeking revenge, wanting to destroy everything, including Ian then, but he's guarding the dark memories from Ian which is a form of protection.

AN ACTUAL MASTERPIECE.
Full album on October 6th. The figure cloaked in black at the end is MIITO, the protagonist of the first two albums and DPR Ian's alter-ego. All of his music is fully influenced by his mental health issues, most notably bipolar disorder and DID (Dissociated Identity Disorder).
I believe the red doll at the end is a link to the incredible film that he released for his last album which you should really react to.
Yes I plan to react to that film! It was actually gonna be this very video, but I saw this just came out and so I changed my mind haha.
Fun fact - I believe the doll at the end was fan made and gifted to Ian, and he loved it so much that he included it in this video :D

Mr Insanity is ians minds highest form and yes we have storyline going on here which i don't know yet

So this green smoke and the entire scenery was actually on his mind. Mr. Insanity and Mito in here has this concept of who's going to take control (representing a door)
The red doll is Ian or one of his personalities at least :) check out Ribbon in Mito movie part I.
I know that everyone kinda talked about it but
to have more informations and understand
more, lan is Bipolar and also has D.I.D which
is (Dissociative identity disorder), he based
the storyline on both of his disorders. Mito
and Mr. Insanity are not just characters but
personalities they came when he was a child
because of severe trauma before the brain
has time to make 1 personality. lan, Mito, Mr.
Insanity and others personalities if he has
more all have different voices, taste in food,
clothes, knowledge there is things that Mito
is going to know or like but not lan.
Mito is the "Persecutor Alter"
Alter is the medical term for the personalities.
Mito is the one who holds all the trauma
that lan went through so lan doesn't really
remember what happened back then he also protects
lan and Mr. Insanity is the one who is most
likely to do something bad and put him in
'bad situations". Hope you understood a little
