Peter telling the waiter he's dead to him because he's gay and how him and Lois will focus all their attention to the hostess now isn't a joke about the waiter being gay. It's using satire to show how ridiculous Peter sounds saying that. That saying that to a complete stranger sounds just as ridiculous as when parents say that to their own gay children.
