Yes, you are right. For blind review, only main text and ref. can be included. However, In most of the cases, title, abs, names, and affiliations can be added. There is no problem. I generally include all and i have not faced any difficulty. Before submitting the manuscript, we need to see authors guidelines. If it is strictly mentioned for adding only main text with ref., we can follow that, otherwise we can add all.

It depends on journal requirements. Some journals ask to put everything in one doc, others ask to put fig. and tables after ref. Some journals ask a separate copy for tables and figures. I would suggest to see author guidelines before submitting the manuscript.

I use latex. It can be downloaded from the internet. It's free.
