Train brakes work on two principles. Pressure operated or vacuum assisted.
Pressure operated will be disabled when compressed air is not available.
Whereas Vac brakes engage when disconnected from the engine or source of vacuum.
When changing engines , the train guard is supposed to engage the mech brake in the guard room.
Additionally , the people responsible for the engine change out too are required to ensure that brakes are engaged and skids are in place.

It happened because The Driver means the Loco pilot disconnected the Engine from the Train for Engine Reverse but the guard failed to engage vacuum brakes before that and as a result of this the train became brake free... and due to down slope the train rolled for 15 km without Engine... thats it.. After that it was stopped by IRCTC employee.
