What a beautiful song and great video - but I'm just trying to figure out why he was puzzled about his girlfriend in the end and the photo? Maybe I missed something....nevertheless, it was so awesome to see his emotions :)

I'm not sure if I've got this right, but here's what I think it means.
The song is basically about missed opportunities because you're too scared to take the plunge (like not leaving your home town), and living with the regret that comes after.
In this case the guy's regret is not meeting up with the girl he loved at the Berkshires for whatever reason (maybe because he didn't think he had any real chance with her). We see him revisit this shop 'Ames' a few times which indicates he's coming there again and again because it was likely the last place he ever saw her, or maybe where they met. Finally he decides to travel to the place where they were going to meet, and he finds her there... but not really. We see that the house she's living in is run-down from the outside, but fine on the inside, and after being hugged by her we see her face is filled only with grief. This was his dream - to live there with her - but it's all in his head, and she's a phantom. Looking at the photo and the date, and seeing his face in the corner (where there was previously a different guy) indicates that that was what could have been, or would have started, if he'd actually taken the opportunity in the first place.
