Yes Iwa is crass and brutality honest to Oikawa, but how else is Oikawa supposed to understand that he shouldn't be pushing himself as hard as he does, or understand that he is amazing just the way he is; and they live in Japan, their customs are different from others. It's perfectly normal to literally knock some sense into someone being stupid when they're someone close to you which is why you see it all the time in Japanese shows.
