    What does pulsation on the 3rd intercostal space(erb's point) or in the 4th intercostal space(tricuspid valve) suggest respectively?

      in the 4th intercostal space,thrill represents VSD

    I’m suffering from thrills for a while now, so strong that I feel them to the left side of my face, making it hard to sleep. My GP didn’t know what I was talking about and didn’t do any examination apart from an ECG which she said was fine. I went to a cardiologist, who told me “thrills do not exist, I’ve never seen them” and didn’t do this type of examination either. Just looked at ECG which again was fine. Neither listened to my heart with a stethoscope or feel my chest or neck. Any idea of what type of examination I should ask for? These thrills started suddenly (together with a sudden very high blood pressure and sharp pain in the second intercostal space that comes and goes) and I don’t know what to do or ask for.

      You can ask for a echocardiogram and CT angiogram of your aorta, ask your physician to palpate the thrill and AUSCULTATe if any murmers

      @@darkemperor8645 Thanks. Is there any other place in the circular system which can be a source of the very strong thrill? It’s gotten significantly worse, it’s almost impossible to sleep. You can feel it clearly just by putting fingers against my neck, it’s as if the whole carotid artery is vibrating. They did send me for an echocardiogram (done by a lab assistant, who had never heard of thrills either) and I was told they didn’t find anything wrong other than minor regurgitation which I was told shouldn’t create any issues. But the vibrations, pain and high blood pressure are still there. Also, as a side-note, my blood pressure is ~30 systolic and ~20 diastolic higher on my right arm vs the left arm… GP had no idea on follow up, just said “take your meds to bring your pressure down”… Is there anything else you’d recommend other than the CT angiogram?

      @@MDF4072 No, still no diagnosis. Pain and the “thrill” vibration is worse, taking pressure meds at 2x the dosage what they were 12 months go though. Also, pressure is roughly 20/10 higher on the right hand side of the body. Any idea what it could be?

    So why would a doctor feel this, and be surprised by it in hospital, yet run no further tests once ecg and troponin are fine?
    She was surprised the pulses were visible - even stomach. And heave of chest. 🤷‍♂️

      @@timmyrobinson1589 you see I'm still a medical student but it could be normal in thin individuals or people with hyperdynamic circulation as in anaemia
