The dialyzer is connected improperly. The red hansen should be connected with the arterial line and the blue hansen should be connected at the venous line.

did'nt mentioned that you have to rinse the dialyzer before connecting it to the blood line especially it was new...
only if it is reused. single use dialyzers aren't rinsed

If u connect this way, then red end must be on top & blue end below..in order to get counter flow in the dialyser.
