Question: #2: To me, the sentence would seem better if "children" were substituted in place of "a child." #13: Answer D also grammatically correct. Enjoyed the test.

It would be better to use "children" instead of "a child" for grammatical accuracy: "They ... in Paris for 6 years when they were children." The correct answer should be "lived." Regarding question #13, you are also correct that answer D is grammatically accurate: "He watches television regularly." and "He watched television regularly." It should be: "She would have passed the exam if she had studied." We mistakenly used a comma instead of a period at the end.
