This is total hearsay from personal experience (does that count as hearsay?) but my guess is that Mary Jane Heaster likely had a Hynogogic Hallucination. They occur just as you're going to sleep or waking up and I have had exactly this hallucination. A faceless version of my brother was standing next to my bed and whenever I wasn't looking directly at him, his head turned on a swivel. The moment I looked at him, his head would whip around to "look" at me. Fun times.
