    Even though you placed a 5.0, the core is still a 3.3. The goal of wider implants is to have more mechanical strength of the prosthetic components (I think...). The issue I have with this system is that the platform/connection remains the same no matter what core diameter you place. Maybe it’s a 5.0 at the threads, but for a molar you still end up with a 3.3, which under function is more at risk of fracturing. If you have enough space in your ridge to place a 5.0 in soft bone, why don’t you underprep the osteotomy and actually use a 5.0 core diameter implant?

      Newer studies show that you want as much bone as possible surrounding an implant, preferably 2mm or more. Same with soft tissue. That is close to impossible when using an 5,0core implant. So in the long run this is a better solution. A 3,3mm implant should not fracture under load as it should be strong enough.

      My question is….let’s say it’s d3 bone, you need to place a 4.0 implant but the d3 dictates you need to go up to a 5.0 but now my implant cutting are right next to adjacent teeth….what to do, say within the recommend 1.5mm away from adjacent tooth

      In that case you would undersize the osteotomy so you can still place the desired 4.0 implant with good primary stability

      I realize this is an old comment but I’ll chime in anyway. Implant stability is a function of bone to implant contact, which is a function of surface area, and the 5.0 MegaGen in this video has more surface area.
      As far as the smaller core fracturing, I can tell you that I’ve been restoring MegaGen implants for over a decade, and started placing them myself recently, and I haven’t see it happen.
