When i got the "First Time?" Achievment i dint know i would torture every single goredoll that came in front of me.
The only way for you to obtain the fancy achievement is by giving an NPC the fancy Hat when they wear it you will get the achievement

How to get 4 achievements at one: no clip to sky then fall then get crippled then die and get disarmed
you need to not die for the cripled achievement and then you need a weapon to get the disarmed achievement
