I will explain it on three counties:
Germany: In 2021 eurofederalists won elections and soon some of EU members joined to Germans creating federalised EU and other countries have decided to leave this idea and remain Independent
Russia: Russo-Ukrainian war turned to be more catastrophic for Putin than in our timeline so nation and some part of the army decided to remove him from president's chair. Navalny was then elected as a new president of the Russian Republic. After that Putin and Prigozhyn and his Wagner's Group escaped to Kaliningrad creating opositional governmant to Navalny's one. They name it Russian Federation to show that they still have some kind of legitimacy even if they lost it due people's riot. (Navalny and Prigozhyn are still alive because war outcome was different)
Poland: After 2023 elections situation in conutry became intense due victory of controversial PIS party. It was so bad that there was danger of civil war so Polish oposition asked EU for direct intervention. They agreed and started invasion but after Russian Federation attacked Masuria and destroyed some part of city called Łódź by nuclear attack, EU forces has stopped on Warta river because they feared that conflict may escalete to atomic war. Russians also decided to stop their offenive because of lack of resources. After that Poland has collapsed and divided on many warlord states.
It's a meme mod based on TNO so I recommend to not take it seriously
